Retinal issues

Hello doctor, I have myopia of 800 degrees in both eyes and I also have floaters.
Two months ago, I experienced seeing flashes of light when going down the stairs, and I underwent a fundus examination.
The doctor mentioned that my retina is thin but there are no tears.
I was advised to monitor the situation.
Recently, I have noticed that when I close my eyes at night, I see branch-like flashes of light (which last for about one second).
Could this indicate a retinal tear?

A-Yun, 20~29 year old female. Ask Date: 2021/11/14

Dr. Gao Pilin reply Ophthalmology

Hello, Ah Yun: The phenomenon of flashes is caused by the vitreous pulling on the retina.
If the examination shows no tears, it should not be a concern.

Hello,
Thank you for reaching out with your concerns regarding your eye health. Based on your description, it seems you are experiencing several symptoms that warrant careful consideration, especially given your history of high myopia (800 degrees) and the presence of floaters (known as "myodesopsia").
Firstly, the phenomenon of seeing flashes of light, particularly in the context of your high myopia, can be indicative of changes occurring in the vitreous gel that fills the eye. As we age or as a result of myopia, the vitreous can begin to shrink and pull away from the retina, which may lead to the perception of flashes of light. These flashes are often described as "lightning" or "flashes" and can be alarming, especially when they occur suddenly or frequently.

The fact that you are also experiencing floaters, which are typically harmless but can be more pronounced in individuals with myopia, adds another layer to your situation. Floaters are caused by tiny clumps of gel or cells within the vitreous that cast shadows on the retina, and they can become more noticeable when looking at a bright background.

However, your recent experience of seeing tree branch-like flashes when your eyes are closed is concerning. This could potentially indicate a more serious issue, such as retinal tears or detachment. Retinal tears can occur when the vitreous pulls away from the retina, creating a tear that can lead to fluid accumulation underneath the retina, resulting in detachment. Symptoms of retinal detachment may include a sudden increase in floaters, flashes of light, and a shadow or curtain effect over your vision.

Given your history of retinal thinning and the new symptoms you are experiencing, it is crucial to seek immediate attention from an eye care professional. A comprehensive eye examination, including a dilated fundus examination, is necessary to assess the health of your retina and to rule out any serious conditions such as retinal tears or detachment.
In terms of management, if a retinal tear is detected, treatment options may include laser therapy or cryotherapy to seal the tear and prevent detachment. If detachment has occurred, surgical intervention may be required to reattach the retina.

It is also important to monitor your symptoms closely. If you notice any sudden changes in your vision, such as an increase in flashes or floaters, or if you experience a sudden loss of vision, you should seek emergency medical attention immediately.

In summary, while your symptoms could be related to the normal aging process of the vitreous, the presence of flashes, especially in conjunction with your high myopia and retinal thinning, necessitates a thorough evaluation by an eye care specialist. Early detection and intervention are key to preserving your vision and preventing potential complications.

Take care, and I hope you find the answers and relief you need soon.
