Can You Get Pregnant from External Contact? Ovulation and Risks - Obstetrics and Gynecology

Are you pregnant?

Doctor, I have a question.
A few days ago, I was intimate with my girlfriend; we only engaged in mutual masturbation and oral sex.
However, there were a few accidental touches where my glans made contact with the outside of her vagina, but we immediately separated.
There was no ejaculation or penetration involved, just accidental contact.
I'm unsure if there were any secretions on my glans, and it happened to be during her ovulation period, although I'm not sure if ovulation had already occurred.
I took Ella, an emergency contraceptive pill, within 6 hours after the incident, but I'm still feeling anxious.
Is there a possibility of pregnancy in this situation? What are the characteristics of the ovulation period that can help us determine whether ovulation has been completed or if it is currently occurring?

Hi, 20~29 year old female. Ask Date: 2021/11/14

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, if you have taken the emergency contraceptive pill, the chances of pregnancy should be low.
Ovulation is difficult to predict, so it's best to use regular contraception.
Wishing you good health.

When it comes to the question of whether you can get pregnant from external contact, particularly during the ovulation period, it’s important to understand the mechanics of conception and the risks involved.

Firstly, pregnancy occurs when sperm fertilizes an egg. This typically happens through vaginal intercourse, but there are other ways sperm can potentially reach the vagina and fertilize an egg. In your case, you mentioned that there was no ejaculation or penetration, but there was some external contact between the glans of the penis and the vaginal area. This raises the question of whether sperm could have been transferred during that contact.

Sperm can survive outside the body for a short period, but they are quite fragile. They require a moist environment to remain viable. If there was any pre-ejaculate (pre-cum) present on the glans of the penis, it is theoretically possible for sperm to be transferred to the vaginal area during the contact you described. However, the likelihood of this leading to pregnancy is relatively low, especially if there was no direct ejaculation.

Regarding your concern about the timing of ovulation, it’s crucial to understand how to identify when ovulation occurs. Ovulation typically happens around the midpoint of the menstrual cycle, which is about 14 days before the start of the next period for those with a regular cycle. Signs that you may be ovulating include:
1. Changes in Cervical Mucus: Around ovulation, cervical mucus becomes clear, stretchy, and resembles raw egg whites. This change helps sperm travel more easily through the cervix.

2. Basal Body Temperature (BBT): Many women track their BBT, which slightly increases after ovulation due to hormonal changes. A sustained increase in temperature can indicate that ovulation has occurred.

3. Ovulation Pain: Some women experience mild pain or cramping on one side of the abdomen during ovulation, known as mittelschmerz.

4. Increased Libido: Some women notice an increase in sexual desire around ovulation due to hormonal fluctuations.

5. Ovulation Predictor Kits: These kits can detect the surge in luteinizing hormone (LH) that occurs just before ovulation.

Since you mentioned that you took Ella (ulipristal acetate), a type of emergency contraception, within six hours of the incident, this is a significant factor. Ella is effective at preventing pregnancy when taken during the ovulation window, and it works primarily by delaying or inhibiting ovulation. If ovulation has not yet occurred, Ella can be very effective in preventing pregnancy. However, if ovulation has already taken place, the effectiveness may be reduced.

In summary, while there is a theoretical risk of pregnancy from the external contact you described, the actual risk is low, especially given the use of emergency contraception. If you are still feeling anxious, consider taking a pregnancy test a few weeks after the incident for peace of mind. Additionally, if you have further concerns about your reproductive health or contraception options, consulting with a healthcare provider can provide personalized guidance and reassurance.
