Orthodontic extraction and periodontal treatment?

Hello Dr.
Tseng: Currently, the orthodontic plan includes the extraction of three wisdom teeth.
However, last week, a periodontal assessment was conducted, and due to mild periodontal symptoms, periodontal treatment is needed (including root planing, laser therapy, and microscale deep cleaning).
I would like to inquire whether it is appropriate to perform tooth extraction and periodontal treatment simultaneously, or would you recommend prioritizing one over the other?

KK, 30~39 year old female. Ask Date: 2021/11/17

Dr. Zeng Guanghui reply Dentistry

If the wisdom tooth that needs to be extracted has severe periodontal symptoms, including pericoronitis or the presence of abscesses, it is recommended to perform periodontal treatment first before extracting the wisdom tooth.
If there is no improvement, both procedures can be done simultaneously.

More Info

When considering whether to perform tooth extraction and periodontal treatment simultaneously, several factors must be taken into account, including the severity of periodontal disease, the specific teeth involved, and the overall treatment plan. In your case, you mentioned that you have mild periodontal symptoms that require treatment, including root planing and laser therapy, alongside the planned extraction of three wisdom teeth.

Generally, if the wisdom teeth have significant periodontal issues, such as pericoronitis (inflammation around the crown of a partially erupted tooth) or if there is an active infection, it is advisable to address the periodontal condition first. This approach allows for the management of any infection or inflammation that could complicate the extraction process. If the periodontal condition is stable and not causing significant issues, simultaneous treatment may be feasible.

Key Considerations:
1. Severity of Periodontal Disease: If your periodontal disease is mild and manageable, and if the extraction of the wisdom teeth is necessary for your orthodontic treatment, it may be appropriate to proceed with both treatments together. However, if there are signs of active infection or significant inflammation, it is better to prioritize periodontal treatment first.

2. Healing and Recovery: Performing both procedures at the same time can increase the complexity of recovery. After an extraction, the surgical site needs time to heal, and if periodontal treatment is also performed, it may complicate the healing process. This is particularly important if you are undergoing laser therapy or deep cleaning, which can cause additional irritation to the gum tissue.

3. Risk of Complications: Combining these procedures may increase the risk of complications, such as prolonged bleeding or infection. If the extraction site becomes infected, it could hinder the healing of the periodontal treatment as well.

4. Consultation with Your Dentist: It is crucial to have a thorough discussion with your dentist or periodontist about your specific situation. They can assess the condition of your gums, the position of the wisdom teeth, and your overall dental health to provide personalized recommendations.

5. Post-Operative Care: If both procedures are performed together, you will need to be diligent about post-operative care. This includes following all instructions regarding oral hygiene, diet, and any prescribed medications to minimize the risk of complications.

Conclusion:
In summary, while it may be possible to perform tooth extraction and periodontal treatment simultaneously, it is generally recommended to prioritize periodontal treatment if there are any signs of infection or significant periodontal disease. This approach ensures that any underlying issues are addressed before proceeding with extractions, which can lead to better outcomes and a smoother recovery process. Always consult with your dental care provider to determine the best course of action tailored to your specific dental health needs.
