Do I need to take a pregnancy test?

Hello doctor, I got my period on September 6th, and it ended on October 3rd to October 9th.
Normally, I have heavier flow on the first and second days.
During this time, I had intercourse twice without any contraception, but I didn't ejaculate fully inside; it was mostly at the front, and I also expelled some afterward.
Then, from October 26th to October 29th, I had another unexpected period, but the flow over those four days was lighter than my usual menstrual flow! The color was between bright red and orange-red, and there were no blood clots, but I did experience some mild cramps.
As of today, November 18th, I've had intercourse 2 to 3 more times under the same circumstances, without specific contraception and without full ejaculation inside.
The last two instances were on the 14th and 16th.
However, during these days, I suddenly craved sweet and sour snacks, and my appetite has increased compared to usual, but I feel a bit fatigued.
Is there a possibility that I could be pregnant? Should I take a pregnancy test? Also, will I get my period this month? Thank you!

Xiao Meier, 20~29 year old female. Ask Date: 2021/11/18

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, if you suspect you might be pregnant, it's advisable to observe your situation.
You can take a pregnancy test 12 days after general sexual intercourse.
If you do not wish to become pregnant, it's important to use contraception at all times; otherwise, there is still a chance of conception.
Wishing you good health.

Based on your description, it seems you have a few concerns regarding your menstrual cycle and the possibility of pregnancy. Let's break down your situation and provide some insights.

Firstly, your menstrual cycle appears to be somewhat regular, with your last period starting on October 3 and ending on October 9. You mentioned having unprotected intercourse around the time of your cycle, which raises the question of whether you could be pregnant. It’s important to note that sperm can survive in the female reproductive tract for up to five days, so if you had intercourse during your fertile window, there is a possibility of conception.

You also noted that you experienced bleeding from October 26 to October 29, which was lighter than your usual menstrual flow and had a different color. This could be a sign of several things, including implantation bleeding, which can occur when a fertilized egg attaches to the uterine lining. However, it could also be a lighter period or even a sign of hormonal fluctuations. The absence of blood clots and the presence of mild cramps are also factors to consider.

Given that you have had unprotected intercourse and are experiencing unusual bleeding, it would be prudent to take a pregnancy test. Home pregnancy tests are generally reliable and can detect the hormone hCG (human chorionic gonadotropin), which is produced during pregnancy. For the most accurate result, it’s best to take the test after your expected period date, as this allows for higher levels of hCG to build up in your urine.

Regarding your question about whether you will have your period this month, it’s difficult to predict. If you are pregnant, your period will likely be delayed or absent. If you are not pregnant, you may still experience a period, although it could be lighter or heavier than usual due to various factors such as stress, diet, or hormonal changes.

In terms of your cravings for sweet and sour foods and increased appetite, these can be early signs of pregnancy for some women, but they can also be attributed to other factors such as hormonal changes or dietary preferences. Feeling more fatigued than usual can also be a symptom of early pregnancy, but it can result from many other causes, including stress, lack of sleep, or changes in diet.

In conclusion, given your situation, I recommend taking a home pregnancy test to clarify whether you are pregnant. If the test is positive, or if you continue to have unusual bleeding or other concerning symptoms, it would be wise to consult with a healthcare provider for further evaluation and guidance. They can provide you with more personalized advice and support based on your specific circumstances. Remember, it’s always better to seek medical advice when in doubt about your health.
