Hearing a heartbeat in one ear?

Hello, doctor.
I would like to ask you a question.
About three days ago, I suddenly heard a heartbeat sound in my left ear.
It felt like my ear (eardrum) was congested, and I could hear the heartbeat for about ten minutes.
At that moment, I felt a bit of palpitations and was scared, so I checked my heart rate and blood pressure.
I had just eaten, and my heart rate was 105 beats per minute.
This situation only happened that once, but since then, I occasionally hear a very faint buzzing sound in my left ear (similar to an electric current).
However, when I try to block out environmental sounds with my hand or earplugs, I can't hear it anymore.
I have a habit of staying up late and am prone to nasal allergies.
I wonder if this could also be a factor.
Thank you for your clarification, doctor.
Wishing you peace and safety.

Hello, the issues you mentioned could be due to: 1) Eustachian tube dysfunction: This is the most common cause and can occur not only during flying, climbing, or diving, but also when forcefully blowing your nose or bending over.
2) Sudden sensorineural hearing loss.
3) Otitis media: a) Acute otitis media: Often occurs alongside upper respiratory infections (like colds) and sinusitis, and in severe cases, there may be ear discharge.
b) Chronic otitis media with acute exacerbation: This usually occurs when there is a pre-existing perforation in the eardrum, often due to contaminated water or improper ear cleaning, leading to inflammation.
4) Auditory nerve degeneration.
All of the above conditions can be appropriately managed with medication after consulting a healthcare provider.
If you have already undergone a pure-tone audiometry test (which requires headphones in a soundproof room where you respond to sounds, rather than just using a tuning fork), and it has been confirmed that you do not have sudden sensorineural hearing loss, then medication may suffice.
However, if you have not had a pure-tone audiometry test, it is advisable to visit a hospital for evaluation to rule out sudden hearing loss.
The golden treatment window for sudden hearing loss is two weeks, and early treatment yields better results.
Wishing you safety and good health.

Experiencing heartbeat sounds in one ear can be concerning, and it’s understandable that you are seeking clarity on this matter. The phenomenon you described, where you heard a heartbeat sound in your left ear accompanied by a feeling of fullness or pressure, can be attributed to several potential causes.

1. Eustachian Tube Dysfunction: One of the most common reasons for experiencing a sensation of fullness in the ear, along with unusual sounds, is Eustachian tube dysfunction. This tube connects the middle ear to the back of the nose and helps equalize pressure. When it doesn’t function properly, it can lead to a feeling of pressure and may cause sounds to be amplified, including your heartbeat.

2. Pulsatile Tinnitus: The sound of your heartbeat in your ear could also be a form of tinnitus known as pulsatile tinnitus. This condition is characterized by hearing a rhythmic sound that usually matches your heartbeat. It can be caused by various factors, including vascular issues, high blood pressure, or even increased blood flow in the vessels near the ear.

3. Middle Ear Issues: Conditions such as middle ear infections (otitis media) or fluid accumulation can also lead to abnormal sounds in the ear. These conditions can create a sensation of fullness and may alter how sounds are perceived.

4. Acoustic Reflex: The ear has a protective mechanism known as the acoustic reflex, which can cause the muscles in the middle ear to contract in response to loud sounds. If you were in a noisy environment or experienced a sudden loud noise, this reflex might have contributed to the sensation you felt.

5. Anxiety and Stress: The heart rate you mentioned (105 beats per minute) after eating could indicate a stress response. Anxiety can heighten awareness of bodily sensations, including sounds in the ear, leading to a perception of heartbeat sounds.

6. Allergies and Sinus Issues: Given your mention of nasal allergies, it’s worth noting that sinus congestion can affect ear pressure and function. Allergies can lead to inflammation and fluid buildup, which may contribute to the symptoms you are experiencing.

7. Hearing Loss or Auditory Nerve Issues: Although less common, issues related to the auditory nerve or sudden sensorineural hearing loss could also manifest as unusual sounds in the ear. If you have not had a hearing test recently, it may be beneficial to get one to rule out any underlying issues.

Given that your symptoms have only occurred once and have not persisted, it may be reasonable to monitor your condition. However, if you experience recurrent episodes, significant discomfort, or if the symptoms worsen, it would be prudent to consult an ear, nose, and throat (ENT) specialist. They can perform a thorough examination, including hearing tests and possibly imaging studies, to determine the underlying cause of your symptoms.

In the meantime, consider maintaining a healthy lifestyle, managing stress, and avoiding loud noises or prolonged use of earbuds, as these can exacerbate ear issues. If you notice any additional symptoms, such as dizziness, significant hearing loss, or persistent tinnitus, seek medical attention promptly.

In summary, while your experience of hearing a heartbeat in your ear can be alarming, it is often related to benign conditions. However, staying vigilant and consulting with a healthcare professional if symptoms persist is the best course of action. Wishing you good health and peace of mind!
