The sensation of tightness and soreness during an erection of the penis?
Hello doctor, about four or five years ago, I experienced pain and some bleeding during intercourse with my girlfriend.
For the past few years, this hasn't happened, but there was one instance in the last couple of years where I felt pain again, which went away when I changed positions.
I didn't pay much attention to it.
However, during my recent health check-up, I had occult blood in my urine, and this time bacteria were detected, while other aspects of the urine test were normal.
Due to the pandemic, I haven't seen my girlfriend much, only a couple of times, but when we did have intercourse, it was over quickly.
Recently, I've been relying on masturbation to relieve myself.
Last night, I watched adult films but didn't masturbate, yet I experienced involuntary ejaculation in the middle of the night.
In the morning, my penis felt tight and sore when erect, but after urinating, the discomfort subsided.
I've had similar experiences before, but it felt like there was some swelling between my penis and navel.
I've also been dealing with gastrointestinal bloating, and my doctor prescribed medication for that.
Additionally, I seem to have lost my morning erections, and the force of my urine stream isn't very strong, although it feels normal when I urinate.
Could you please advise if these issues are related, or if I just need to rest?

Hello: Your urine shows traces of blood, so you should have it checked by a urologist at the hospital.
At 48 years old, you may have prostate issues that should also be evaluated.
Wishing you good health.

More Info
Understanding painful erections and related symptoms can be complex, as they may stem from various physical and psychological factors. Based on your description, it seems you have experienced pain during erections, changes in sexual performance, and some urinary symptoms. Here’s a comprehensive overview of potential causes and solutions.
Potential Causes
1. Physical Issues:
- Infections: The presence of bacteria in your urine suggests a possible urinary tract infection (UTI) or another type of infection affecting the genital area. Infections can lead to inflammation, which may cause pain during erections.
- Peyronie's Disease: This condition involves the development of fibrous scar tissue inside the penis, leading to painful erections and curvature. While you haven't mentioned curvature, the pain could be indicative of this condition.
- Hormonal Changes: The absence of morning erections (morning wood) and decreased urinary stream could suggest hormonal imbalances, particularly low testosterone levels. This can affect libido and erectile function.
- Pelvic Floor Dysfunction: Tension or dysfunction in the pelvic floor muscles can lead to pain during erections and discomfort in the genital area.
2. Psychological Factors:
- Anxiety and Stress: Performance anxiety, especially after experiencing pain during intercourse, can lead to a cycle of anxiety that affects sexual performance. This can manifest as premature ejaculation or difficulty maintaining an erection.
- Depression: Emotional health plays a significant role in sexual function. Feelings of sadness or disinterest can lead to decreased libido and performance issues.
3. Lifestyle Factors:
- Sedentary Lifestyle: Lack of physical activity can contribute to poor blood circulation, which is essential for erectile function.
- Diet and Hydration: Poor nutrition and dehydration can affect overall health, including sexual health.
Solutions and Recommendations
1. Medical Evaluation:
- It is crucial to consult a healthcare provider, preferably a urologist, to discuss your symptoms in detail. They may recommend tests such as blood work to check hormone levels, imaging studies, or a physical examination to assess for any structural issues.
2. Treatment for Infections:
- If an infection is confirmed, appropriate antibiotics or treatments will be necessary. Ensure you complete the full course of any prescribed medication.
3. Addressing Hormonal Issues:
- If low testosterone is identified, your doctor may discuss hormone replacement therapy or other interventions to help restore normal levels.
4. Pelvic Floor Therapy:
- Consider seeing a physical therapist who specializes in pelvic floor dysfunction. They can provide exercises to strengthen and relax the pelvic muscles, which may alleviate pain and improve function.
5. Psychological Support:
- Engaging in therapy or counseling can help address any underlying anxiety or stress related to sexual performance. Cognitive-behavioral therapy (CBT) is particularly effective in managing performance anxiety.
6. Lifestyle Modifications:
- Incorporate regular physical activity into your routine to improve circulation and overall health.
- Maintain a balanced diet rich in fruits, vegetables, whole grains, and lean proteins to support hormonal balance and general well-being.
- Stay hydrated and limit alcohol and tobacco use, as these can negatively impact erectile function.
7. Communication with Partners:
- If you are in a relationship, open communication with your partner about your experiences and feelings can help alleviate some of the pressure and anxiety associated with sexual performance.
Conclusion
Painful erections and associated symptoms can significantly impact your quality of life and emotional well-being. It is essential to take these symptoms seriously and seek professional medical advice. By addressing both the physical and psychological aspects of your condition, you can work towards finding effective solutions and improving your sexual health. Remember, you are not alone in this, and help is available.
