Palpitations, shortness of breath, and diarrhea?
Hello Dr.
Wu,
Since May, I have been experiencing hyperventilation and arrhythmia with blood pressure readings of 140/9X.
I underwent several ECGs in May, which showed abnormal results at times and normal results at others.
A cardiac ultrasound performed once returned normal results, and in September, I had a stress test ECG that was also normal.
I take 1.25 mg of Concor and 40 mg of Datanol daily, and I had never taken similar medications before.
By the end of October, my blood pressure readings were at their lowest, around 9X/5X.
After that, I engaged in exercise and intermittent fasting, losing about 10 kg.
Since July, I have not experienced hyperventilation or palpitations.
Recently, on December 1, I received my second dose of the BNT vaccine, and starting around December 3, I began to experience severe palpitations (feeling like my heart and scalp are vibrating).
I have been unable to sleep well at night, and during the day, I sometimes feel palpitations while sitting or standing, which have persisted for six days.
Around December 2, I started having daily bowel movements, mostly diarrhea, and occasionally experiencing chest pain.
On December 8, I consulted a cardiologist who did not perform an X-ray or blood tests.
The doctor advised me to take my cardiovascular medications at night and prescribed Xanax, but I still wake up due to persistent strong heartbeats and the sensation of my head vibrating.
P.S.
I also experienced palpitations after my first BNT dose, and when I consulted the same doctor, blood tests showed no abnormalities, and the doctor advised me not to be too anxious.
I would like to ask if I should go to a major hospital for further examination, as I have woken up several times at night feeling the urge to go to the emergency room, but I worry that it might just be my anxiety causing this, which would waste medical resources.
Thank you for your advice.

When it comes to understanding palpitations and anxiety, it is essential to recognize the interplay between psychological and physiological factors. Your experience of palpitations, particularly after receiving the second dose of the BNT vaccine, along with episodes of hyperventilation and elevated blood pressure, suggests that both anxiety and potential cardiac issues may be at play.
Palpitations are often described as the sensation of feeling your heart beating, which can be perceived as racing, fluttering, or pounding. They can be triggered by various factors, including anxiety, stress, caffeine, certain medications, and underlying heart conditions. In your case, the timing of the palpitations following the vaccination could indicate a stress response, especially if you have a history of anxiety.
Your medical history indicates that you have undergone several cardiac evaluations, including multiple electrocardiograms (ECGs) and an echocardiogram, which have returned normal results. This is reassuring, as it suggests that there may not be a significant underlying cardiac pathology. However, the intermittent nature of your symptoms and their correlation with anxiety episodes cannot be overlooked. Anxiety can lead to physical symptoms such as palpitations, chest pain, and gastrointestinal disturbances, including diarrhea, which you have reported.
Given your recent symptoms, including severe palpitations and difficulty sleeping, it is advisable to seek further evaluation. While your previous tests have been normal, the persistence and severity of your current symptoms warrant a thorough reassessment. Visiting a larger hospital or a specialized cardiac center may provide you with access to more comprehensive diagnostic tools, such as continuous Holter monitoring, which can capture your heart's rhythm over an extended period, or even an electrophysiological study if indicated.
In terms of medication, you mentioned taking a combination of antihypertensive medications and experiencing side effects. It is crucial to communicate these side effects to your healthcare provider, as they may need to adjust your treatment regimen. The sensation of your heart "vibrating" or "shaking" could be related to anxiety or the effects of medication, but it is essential to rule out any cardiac causes.
Additionally, the psychological aspect of your symptoms should not be underestimated. Anxiety can exacerbate physical symptoms, leading to a cycle of worry and increased physiological responses. Cognitive-behavioral therapy (CBT) has been shown to be effective in managing anxiety and can help you develop coping strategies to deal with your symptoms.
In summary, while your cardiac evaluations have been reassuring, the persistence of your symptoms, particularly after vaccination, suggests that further evaluation is warranted. It is essential to address both the physical and psychological components of your health. Do not hesitate to seek care from a cardiologist, and consider discussing your anxiety with a mental health professional. Your health and well-being are paramount, and it is always better to err on the side of caution when it comes to your heart.
