Shortness of breath?
Dear Dr.
Tsai: I have been experiencing symptoms of a cold, including cough and sneezing, since early May.
I took medication but did not fully recover.
By the end of May, I noticed that both sides of my nose were completely congested, making it difficult to breathe and forcing me to breathe through my mouth.
Additionally, I felt phlegm in my throat.
I consulted a doctor who diagnosed me with allergic rhinitis, but after taking the medication twice, it had no effect.
I then visited a larger hospital's ENT department, where the doctor diagnosed me with chronic pharyngitis.
Initially, the medication was effective, but after taking it for 5 or 6 times and then stopping, my throat symptoms returned, and I felt the need to cough.
I also experienced slight post-nasal drip, severe nasal congestion (only one side is blocked while sleeping, alternating sides), and during the day, if I stand and move around, I have less congestion, but when sitting still, it becomes very congested.
I also feel tightness in my chest, and if I breathe forcefully while congested, it causes chest pain.
Could you please advise me on what this condition might be and how to treat it?

Dr. Cai Zhenggu reply Otolaryngology
Hello Mr.
Hsieh: Based on your description, your issue is chronic pharyngitis caused by mouth breathing due to nasal congestion.
I recommend that you come to my clinic for an examination, as there are many potential causes of nasal congestion.
A thorough evaluation is necessary to determine the underlying cause of your nasal obstruction before deciding on a treatment plan.

More Info
Breathing difficulties can be a complex issue, often stemming from various underlying conditions. Based on your description, it seems that you are experiencing a combination of chronic rhinitis and throat inflammation, which can significantly affect your quality of life. Let’s break down your symptoms and explore potential treatments.
Understanding Your Symptoms
1. Chronic Rhinitis: This condition is characterized by persistent nasal congestion, sneezing, and runny nose, often due to allergies or irritants. In your case, the nasal congestion that alternates between nostrils and worsens at night suggests that you might be dealing with allergic rhinitis or even a structural issue like a deviated septum.
2. Throat Inflammation: The sensation of phlegm in your throat and the need to cough can indicate chronic pharyngitis or laryngitis, often linked to post-nasal drip, where mucus from the nasal passages drips down the back of the throat. This can lead to irritation and inflammation, causing discomfort and a persistent cough.
3. Chest Tightness and Pain: The tightness in your chest, especially when breathing heavily or during episodes of nasal congestion, could be related to several factors. It may be a result of anxiety, which can manifest as hyperventilation, or it could be due to bronchial irritation from post-nasal drip or even asthma, particularly if you have a history of respiratory issues.
Potential Causes
- Allergic Rhinitis: This is often triggered by allergens such as pollen, dust mites, or pet dander. It can lead to chronic nasal congestion and throat irritation.
- Chronic Sinusitis: If your nasal passages are inflamed for an extended period, it can lead to sinusitis, which may cause thick mucus and contribute to your throat symptoms.
- Gastroesophageal Reflux Disease (GERD): Sometimes, acid reflux can cause throat irritation and a sensation of mucus in the throat, which can be mistaken for post-nasal drip.
Treatment Options
1. Medications:
- Antihistamines: These can help alleviate symptoms of allergic rhinitis. If over-the-counter options are ineffective, your doctor may prescribe stronger antihistamines.
- Nasal Corticosteroids: These are effective in reducing inflammation in the nasal passages and can help with both nasal congestion and post-nasal drip.
- Expectorants: Medications like guaifenesin can help thin mucus, making it easier to expel and potentially relieving throat discomfort.
2. Nasal Irrigation: Using a saline nasal spray or a neti pot can help clear mucus and allergens from your nasal passages, providing immediate relief from congestion.
3. Lifestyle Modifications:
- Avoid Triggers: Identify and avoid allergens or irritants that worsen your symptoms.
- Stay Hydrated: Drinking plenty of fluids can help thin mucus and keep your throat moist.
- Humidifier: Using a humidifier in your bedroom can help keep the air moist, which may alleviate nasal congestion and throat irritation, especially at night.
4. Follow-Up Care: Since your symptoms have persisted despite treatment, it is crucial to follow up with your healthcare provider. They may recommend further evaluation, such as allergy testing or imaging studies, to rule out other conditions like sinusitis or structural abnormalities.
5. Referral to Specialists: If your symptoms do not improve, consider seeing an allergist or an ear, nose, and throat (ENT) specialist for a more comprehensive evaluation and tailored treatment plan.
Conclusion
Breathing difficulties can stem from a variety of causes, and it is essential to approach treatment holistically. By addressing both the nasal and throat symptoms, you can work towards finding relief. Regular follow-ups with your healthcare provider will be key in managing your condition effectively. Remember, it’s important to communicate openly about your symptoms and treatment responses to ensure the best possible care.
