My throat, nose, and ears are all very uncomfortable?

Hello, Doctor: Thank you for your previous response.
For the past few days, I have been experiencing phlegm in my throat, which is very uncomfortable.
I keep clearing my throat, but each time it hurts.
My voice has also become a bit hoarse.
Additionally, I have allergic rhinitis, and it seems to have worsened; my nasal discharge has turned yellow-green, thick, and foul-smelling.
When I blow my nose, my ears hurt.
I can't even smell when I burn food while cooking.
(I have been breathing through my mouth for a long time due to severe nasal congestion, which makes my throat very dry.) Others say my breathing sounds loud, but I can't hear it myself.
On a side note, my family smokes, so I am forced to inhale secondhand smoke all day, and I know my allergic rhinitis won't improve, so I haven't been taking any medication.
Furthermore, I have been feeling very dizzy lately.
Are my symptoms indicative of a cold, or are they complications from allergic rhinitis? Does excessive exposure to secondhand smoke worsen allergic rhinitis? Why do others say my breathing sounds loud, yet I can't hear it? Why is there a loud noise when I sleep? Does my condition require surgery? If so, what is the surgical process like? Will it recur after surgery? If left untreated, will it resolve on its own, or could complications arise? I live in Taoyuan City; should I visit a small clinic or a large hospital? Which hospitals or clinics are recommended? I apologize for asking so many questions.
Thank you for your response.

Hello: Based on your description, you may have inflammation of the throat and sinuses.
Whether it is a complication of a cold or allergic rhinitis can only be determined through a clinical examination.
Inhaling excessive secondhand smoke typically exacerbates allergic rhinitis and related issues such as loud breathing.
It is recommended that you consult an otolaryngologist at a teaching hospital for evaluation and inquiries.
Sincerely, Huxin.

Your symptoms suggest a complex interplay between allergic rhinitis, potential sinusitis, and possibly eustachian tube dysfunction. Let's break down your concerns and provide some insights into each aspect.

Understanding Your Symptoms
1. Throat Discomfort and Mucus Production: The presence of phlegm in your throat, along with pain when clearing your throat, indicates that your body is responding to an irritant or infection. This could be due to post-nasal drip, which is common in individuals with allergic rhinitis or sinusitis. The yellow-green color of your nasal discharge suggests a possible bacterial infection, especially if accompanied by a foul odor.

2. Nasal Congestion and Ear Pain: The pain in your ears when blowing your nose is likely due to eustachian tube dysfunction. The eustachian tube connects the middle ear to the back of the nose and helps equalize pressure. When you have nasal congestion, this tube can become blocked, leading to pressure and pain in the ears.

3. Impact of Secondhand Smoke: Exposure to secondhand smoke can indeed exacerbate allergic rhinitis and other respiratory conditions. It can irritate the nasal passages and throat, leading to increased mucus production and inflammation. This could explain the worsening of your symptoms, especially if you are frequently exposed to smoke.

4. Hearing Your Breathing: The perception of loud breathing sounds, which you cannot hear, may be related to your nasal congestion. When the nasal passages are blocked, you may be more aware of the sounds produced by your breathing, particularly if you are breathing through your mouth.

5. Dizziness: The dizziness you are experiencing could be related to several factors, including sinus pressure affecting your inner ear, dehydration, or even anxiety related to your discomfort.

Potential Diagnosis
Your symptoms could be indicative of:
- Acute Sinusitis: This condition often follows a cold or allergic reaction and can cause thick, discolored nasal discharge, facial pain, and ear discomfort.

- Chronic Allergic Rhinitis: Persistent nasal symptoms due to allergies can lead to complications like sinusitis.

- Eustachian Tube Dysfunction: This can result from nasal congestion and can lead to ear pain and pressure.

Treatment Options
1. Medications: Over-the-counter antihistamines, decongestants, and nasal corticosteroids may help alleviate your symptoms. If you suspect a bacterial infection, a healthcare provider may prescribe antibiotics.

2. Avoiding Irritants: Reducing exposure to secondhand smoke and other allergens can significantly improve your symptoms.

3. Hydration and Humidification: Staying hydrated and using a humidifier can help thin mucus and ease throat discomfort.

4. Nasal Irrigation: Saline nasal sprays or rinses can help clear nasal passages and reduce congestion.

When to Seek Further Care
If your symptoms persist or worsen, it is advisable to consult an Ear, Nose, and Throat (ENT) specialist. They can perform a thorough examination, possibly including imaging studies, to determine if there are structural issues or chronic sinusitis that may require more intensive treatment or surgery.

Surgical Considerations
If surgery is deemed necessary (for example, for chronic sinusitis or structural issues), it typically involves procedures like endoscopic sinus surgery. This minimally invasive surgery aims to clear blocked sinuses and improve drainage. The recurrence of symptoms post-surgery can vary, but many patients experience significant relief.

Conclusion
Given the complexity of your symptoms, it would be wise to visit a larger medical center or specialized ENT clinic in Taoyuan City for a comprehensive evaluation. Hospitals like Taoyuan General Hospital or other reputable ENT clinics would be suitable options. They can provide the necessary diagnostic tests and tailored treatment plans to address your concerns effectively.

Please remember that while some conditions may resolve on their own, others may lead to complications if left untreated. Seeking medical advice sooner rather than later is always a prudent choice.
