Post-Surgery Concerns: Discharge After Fistula Surgery - Surgery

Post-fistula surgery

Hello Doctor, I had anal fistula surgery on November 19th and my last follow-up appointment was on December 13th.
The doctor said that the wound has healed about 90% and that I do not need to return for further visits.
However, as of today, December 27th, I am still experiencing a small amount of discharge.
Is this normal? Or does the external wound need to be completely healed before there will be no more discharge?

Xiao Wen, 20~29 year old female. Ask Date: 2021/12/27

Dr. Ke Fangxu reply Surgery

Typically, one must wait until the wound is completely healed to avoid any discharge.
The time required for complete healing varies depending on the size of the surgical wound; larger wounds may take up to two months to heal.
If there is still discharge after three months, it is advisable to consult a physician to determine if it is normal.

After undergoing fistula surgery, it is not uncommon to experience some discharge from the surgical site even weeks after the procedure. In your case, since you had the surgery on November 19 and the doctor indicated that the wound was 90% healed by your last visit on December 13, the presence of a small amount of discharge as of December 27 can still be considered within the realm of normal healing.

Fistula surgeries, particularly those involving the anal region, can be complex due to the nature of the tissue involved and the potential for residual inflammation or fluid accumulation. The healing process can vary significantly from person to person, influenced by factors such as the extent of the surgery, individual healing capacity, and any underlying health conditions.
Typically, the discharge you are experiencing could be due to several reasons:
1. Normal Healing Process: After surgery, the body goes through a healing phase where it may produce some fluid as part of the inflammatory response. This fluid can be a mix of blood, lymph, and other bodily fluids, which is a natural part of the healing process.

2. Residual Infection: While it is less likely if your doctor has assessed the wound as healing well, any lingering infection could lead to discharge. Signs of infection include increased redness, swelling, warmth, or a foul odor from the discharge. If you notice any of these symptoms, it would be prudent to consult your physician.

3. Granulation Tissue: As the wound heals, granulation tissue forms, which can sometimes produce a small amount of discharge. This tissue is part of the healing process and is generally a good sign that the body is repairing itself.

4. Wound Care: Proper wound care is crucial for healing. If the area is not kept clean or if there is friction from clothing, it can lead to irritation and discharge. Ensure that you are following any post-operative care instructions provided by your surgeon.

Regarding your question about whether the discharge will cease once the outer wound is fully healed, it is possible. However, the internal healing process may take longer than the external appearance of the wound suggests. The outer skin may close up before the underlying tissues have completely healed, which can lead to continued discharge until everything is fully resolved.

If the discharge persists beyond a few weeks or if you have any concerns about the nature of the discharge, it is advisable to follow up with your healthcare provider. They may want to assess the wound again to ensure that there are no complications and that the healing process is proceeding as expected.

In summary, while some discharge can be normal after fistula surgery, it is essential to monitor the situation closely. If you experience any concerning symptoms or if the discharge does not improve, do not hesitate to reach out to your healthcare provider for further evaluation and guidance. Your health and peace of mind are paramount, and it is always better to err on the side of caution when it comes to post-surgical recovery.
