Coccygeal issues

Hello, doctor! I've been experiencing pain in my coccyx for about 2 years.
In September 2020, I consulted a doctor who mentioned that it seemed like a ligament was strained, but there was no improvement.
In October 2021, I went for another consultation and was told there was a slight displacement of the pelvis.
I underwent physical therapy and took medication, but the pain persists and seems to be getting worse.
What could be the problem, doctor?

Dall, 10~19 year old female. Ask Date: 2022/01/01

Dr. Shi Guozheng reply Orthopedics

Sometimes, inflammation of the tissues near the coccyx can be treated with medication.
If there is no improvement, identifying the pain point and injecting corticosteroids may provide some relief.

More Info

Coccydynia, or tailbone pain, is a condition that can significantly impact a person's quality of life. It is characterized by pain in the coccyx, which is the small bone at the base of the spine. Understanding the causes, symptoms, and treatment options for coccydynia is crucial for effective management.

Causes of Coccydynia
Coccydynia can arise from various factors, including:
1. Trauma or Injury: A fall onto the tailbone or prolonged sitting on a hard surface can lead to inflammation or injury of the coccyx.

2. Repetitive Strain: Activities that involve repetitive movements or prolonged sitting can strain the muscles and ligaments around the coccyx.

3. Childbirth: Women may experience coccydynia after childbirth due to the pressure exerted on the coccyx during delivery.

4. Infections or Tumors: Although rare, infections or tumors in the area can cause pain.

5. Postural Issues: Poor posture or pelvic alignment can contribute to coccyx pain.

Symptoms of Coccydynia
The primary symptom of coccydynia is localized pain at the tailbone, which may be exacerbated by sitting, standing, or transitioning from sitting to standing. Other symptoms may include:
- Pain that worsens with prolonged sitting or standing.

- Discomfort when moving from sitting to standing.

- Tenderness when pressure is applied to the coccyx.

- Pain during bowel movements or sexual intercourse.

Treatment Options for Coccydynia
Treatment for coccydynia often involves a combination of self-care measures, physical therapy, and medical interventions. Here are some common approaches:
1. Self-Care Measures:
- Cushions: Using a cushion or a "donut" pillow can relieve pressure on the coccyx while sitting.

- Heat Therapy: Applying heat to the affected area can help reduce pain and inflammation.

- Avoiding Prolonged Sitting: Taking breaks to stand and move around can alleviate discomfort.

2. Physical Therapy: A physical therapist can provide exercises to strengthen the pelvic floor and improve posture, which may help alleviate pain.

3. Medications:
- Nonsteroidal Anti-Inflammatory Drugs (NSAIDs): Over-the-counter medications like ibuprofen or naproxen can help reduce pain and inflammation.

- Corticosteroid Injections: In some cases, corticosteroid injections may be administered to reduce inflammation around the coccyx.

4. Surgery: In rare cases where conservative treatments fail, surgical options such as coccygectomy (removal of the coccyx) may be considered.

Conclusion
Given your history of tailbone pain for over two years, it is essential to revisit your healthcare provider for a thorough evaluation. Persistent pain, especially if it is worsening, may require further diagnostic imaging, such as X-rays or MRI, to rule out any underlying issues like fractures, infections, or structural abnormalities. A multidisciplinary approach involving pain management specialists, physical therapists, and possibly orthopedic surgeons may be beneficial in managing your condition effectively.

In summary, coccydynia can be a challenging condition to treat, but with the right combination of therapies and lifestyle adjustments, many individuals find relief. It is crucial to communicate openly with your healthcare provider about your symptoms and treatment progress to tailor an effective management plan.
