Itchy nipples may be suspected to be due to a pustule?

I have been experiencing itching of the nipples for nearly two years, with some discharge and mild itching, but recently the itching has become so severe that I can't sleep.
I have no previous history of nipple issues and have used eczema cream for a while without any effect.
To maintain cleanliness, I used a breast pump to draw out the nipples and noticed a slight white area beneath the skin, and I'm unsure if this is an abscess.
Please help me with this.
Thank you.

Based on your description, it does seem to be an abscess.
However, since you are already 61 years old, it would be advisable to undergo a breast examination.

Itchy nipples can be a distressing symptom, and understanding the potential causes and solutions is crucial for addressing the issue effectively. Based on your description, it seems that you have been experiencing itching and discharge from your nipples for about two years, with recent symptoms worsening to the point of affecting your sleep. You also mentioned using a cream for eczema without success and observing a white appearance beneath the skin of your nipples, which raises concerns about possible infection or other underlying conditions.

There are several possible causes for itchy nipples, including:
1. Eczema or Dermatitis: This is a common skin condition that can cause itching, redness, and irritation. If you have a history of eczema, it may be exacerbating your symptoms. Topical corticosteroids or emollients are often prescribed for relief, but if the condition persists, a dermatologist may need to evaluate your treatment plan.

2. Infections: Bacterial or fungal infections can lead to symptoms like itching and discharge. The presence of a white appearance under the skin could suggest a possible infection, such as a sebaceous cyst or an abscess. If you suspect an infection, it is important to seek medical attention for appropriate treatment, which may include antibiotics or drainage of any abscess.

3. Hormonal Changes: Hormonal fluctuations, particularly those related to the menstrual cycle, pregnancy, or breastfeeding, can lead to changes in the breast tissue, resulting in itching or sensitivity. If you are experiencing other symptoms related to hormonal changes, it may be worth discussing with your healthcare provider.

4. Allergic Reactions: Allergies to soaps, detergents, or fabrics can cause localized itching. Consider any new products you may have introduced into your routine that could be irritating your skin.

5. Paget's Disease of the Nipple: Although rare, this condition can cause itching, redness, and discharge from the nipple. It is associated with underlying breast cancer, so if your symptoms persist or worsen, it is essential to have a thorough evaluation by a healthcare professional.

To address your symptoms, here are some steps you can take:
- Consult a Healthcare Provider: Given the duration and severity of your symptoms, it is advisable to see a healthcare provider, preferably a dermatologist or a breast specialist. They can perform a physical examination, possibly order imaging or lab tests, and provide a definitive diagnosis.

- Avoid Irritants: Identify and avoid any potential irritants, such as harsh soaps, lotions, or fabrics that may exacerbate your symptoms.

- Maintain Hygiene: Keep the area clean and dry. If you are using a breast pump, ensure that it is clean and that you are using it correctly to avoid irritation.

- Topical Treatments: If eczema is confirmed, your doctor may prescribe a stronger topical treatment than what you have used previously. Over-the-counter hydrocortisone cream may provide temporary relief, but it is best to consult a doctor for a tailored treatment plan.

- Monitor Symptoms: Keep a diary of your symptoms, noting any changes or triggers. This information can be helpful for your healthcare provider in diagnosing the issue.

In summary, itchy nipples can arise from various causes, and it is essential to seek medical advice for a proper diagnosis and treatment plan. Given your symptoms, especially the discharge and the appearance of white under the skin, a thorough evaluation is warranted to rule out any serious conditions and to find an effective solution to alleviate your discomfort.
