Experiencing palpitations every night before sleep after receiving the vaccine?

Hello Director Wu,
I received my second dose of the Moderna vaccine on December 30.
That same evening, I experienced a noticeable increase in my heart rate, ranging from 100 to 120 beats per minute.
That night, I took medication to lower my heart rate and for anxiety.
The next morning, I went to the emergency room, where blood tests, an electrocardiogram (ECG), and X-rays indicated that I did not have myocarditis or pericarditis.
However, the doctor mentioned that while my X-ray and ECG were normal, the blood test showed signs of infection or inflammation, though it was uncertain if this was vaccine-related.

Since then, I have experienced palpitations every night while lying in bed, with my heart rate around 90 to 110 beats per minute for about 10 days.
I have had trouble sleeping, so I took medications such as Jing An Ning and a beta-blocker.

I would like to ask, given that I have palpitations every night but no symptoms of chest tightness or shortness of breath, do I need to go to the hospital for further examination? I am concerned that the inflammation may not have been detected on the second night and may have started afterward.
(I have a history of autonomic nervous system dysregulation with tachycardia and panic disorder symptoms.)
Thank you, Director, and I wish you good health.

If you experience persistent chest tightness or difficulty breathing within 28 days after receiving the vaccine, it is recommended to visit a hospital for further evaluation.

Experiencing palpitations after receiving a COVID-19 vaccine, such as the Moderna vaccine, can be concerning, especially if you have a history of anxiety or autonomic nervous system disorders. It's important to understand the context of your symptoms and when to seek further medical help.

Firstly, it's not uncommon for individuals to experience transient side effects after vaccination, including palpitations, especially if they have underlying anxiety or autonomic dysregulation. The body's response to the vaccine can include an increase in heart rate due to the immune response, stress, or anxiety about the vaccination itself. In your case, the elevated heart rate you experienced (100-120 beats per minute) shortly after vaccination could be attributed to a combination of these factors.

You mentioned that you sought emergency care and underwent tests, including blood tests, an electrocardiogram (ECG), and chest X-rays, which all returned normal results. This is reassuring, as it suggests that there are no acute cardiac issues such as myocarditis or pericarditis, which have been reported but are rare side effects of mRNA vaccines. The presence of inflammation or infection in your blood tests could indicate a mild reaction to the vaccine or another unrelated issue.

Given that you continue to experience palpitations at night but without accompanying symptoms like chest pain or shortness of breath, it may not be immediately necessary to return to the hospital unless your symptoms worsen or new concerning symptoms arise. However, it is crucial to monitor your condition closely. If the palpitations persist, worsen, or are accompanied by other symptoms, such as dizziness, fainting, or significant anxiety, you should seek medical attention.

In the meantime, consider the following strategies to help manage your symptoms:
1. Relaxation Techniques: Engage in relaxation exercises such as deep breathing, meditation, or yoga. These can help reduce anxiety and may alleviate palpitations.

2. Sleep Hygiene: Ensure you have a calming bedtime routine. Avoid stimulants like caffeine and nicotine, especially in the hours leading up to bedtime.

3. Hydration and Nutrition: Stay well-hydrated and maintain a balanced diet. Sometimes, dehydration or low blood sugar can contribute to palpitations.

4. Follow-Up with Your Doctor: Since you have a history of autonomic nervous system issues and panic disorder, it may be beneficial to follow up with your primary care physician or a cardiologist. They can provide further evaluation and reassurance, and if necessary, adjust any medications you are taking for anxiety or heart rate control.

5. Mental Health Support: Given your history of panic disorder, consider speaking with a mental health professional. They can help you develop coping strategies for anxiety and monitor your mental health in the context of your physical symptoms.

In conclusion, while your symptoms are concerning, the normal test results are reassuring. Continue to monitor your condition, practice self-care, and do not hesitate to seek medical advice if your symptoms change or worsen. Your health and peace of mind are paramount, and addressing both physical and mental health aspects is essential for your overall well-being.
