Palpitations after injection?
Hello Doctor: I do not have any diseases or drug allergies.
Although I have mild nasal allergies, I have previously experienced allergic reactions to shrimp and sudden hives, but those incidents occurred only once or twice several years ago, and I generally do not have any unusual symptoms.
Today, my mother went to see an orthopedic doctor for a sprained ankle and received an anti-inflammatory injection intravenously.
About 3 to 4 hours after returning home from the clinic, she felt an uncomfortable sensation of a rapid heartbeat, but after lying down and resting for a few hours, she felt fine.
This reminded me of an incident earlier this year when I had a cold and sore throat.
The doctor at the clinic also suggested an injection for quicker recovery, which I received in my arm.
After returning home, I also experienced a rapid heartbeat and discomfort about 3 to 4 hours later.
I went back to see the same doctor, who confirmed that my heart rate was indeed elevated but advised me to rest lying down, and after following his instructions, I felt fine after a few hours.
The similarities between my experience and my mother's raise some questions for me.
Neither of us has drug allergies or any special diseases, and we saw different doctors at the clinic.
Why did we both experience this situation? Is there any danger, and should we undergo any tests? If we are eligible to receive the COVID-19 vaccine in the future, can we still get vaccinated? Since we do not have a regular family doctor, I would appreciate your assistance in answering these questions.
Thank you.

Dr. Wu Zongying reply Family Medicine
Hello: You asked about the cause of palpitations after vaccination.
On one hand, it is a response from the autonomic nervous system; on the other hand, the medication entering the body can also trigger some reactions, which will gradually subside.
These reactions should not be classified as allergic reactions.
There is no need to fear receiving the COVID-19 vaccine because of this; if you do not have an allergy to the vaccine, you should actually get vaccinated against COVID-19.

Post-injection palpitations can be concerning, especially when they occur after receiving an injection, such as an anti-inflammatory medication. It's understandable to feel anxious about these symptoms, particularly when they have happened to both you and your mother after different injections. Let's explore the potential causes, concerns, and recommendations regarding this situation.
Firstly, it's important to recognize that palpitations, or the sensation of a rapid or irregular heartbeat, can occur for various reasons. In the context of receiving an injection, there are a few possible explanations:
1. Medication Reaction: Some medications can cause side effects, including palpitations. Non-steroidal anti-inflammatory drugs (NSAIDs) and corticosteroids, commonly used for inflammation, can sometimes lead to increased heart rate as a side effect. While you mentioned that neither you nor your mother has a history of drug allergies, it's still possible to experience side effects from medications that are not classified as allergens.
2. Anxiety and Stress Response: The experience of receiving an injection can induce anxiety in some individuals. This anxiety can trigger a physiological response, including increased heart rate and palpitations. If you or your mother felt anxious about the injection or the underlying condition being treated, this could explain the symptoms.
3. Vasovagal Response: Some individuals may experience a vasovagal response, which can occur during or after medical procedures. This response can lead to a sudden drop in heart rate and blood pressure, followed by a compensatory increase in heart rate, resulting in palpitations.
4. Dehydration or Electrolyte Imbalance: If either of you were dehydrated or had an electrolyte imbalance prior to the injection, this could also contribute to palpitations. It's essential to stay well-hydrated, especially when undergoing medical treatments.
Regarding your concerns about the safety of future vaccinations, including the COVID-19 vaccine, it's crucial to consult with a healthcare provider. Generally, having experienced mild palpitations after injections does not preclude you from receiving vaccines. However, it is advisable to inform the healthcare provider about your previous experiences so they can monitor you appropriately during the vaccination process.
If the palpitations persist or are accompanied by other concerning symptoms, such as chest pain, shortness of breath, or dizziness, it would be wise to seek medical attention. A healthcare professional may recommend tests such as an electrocardiogram (ECG) or a Holter monitor to assess your heart's rhythm and rule out any underlying conditions.
In summary, while post-injection palpitations can be alarming, they are often benign and can result from various factors, including medication effects, anxiety, or physiological responses. It's essential to communicate any concerns with a healthcare provider, especially regarding future vaccinations. They can provide personalized guidance based on your medical history and current health status. Always prioritize open communication with healthcare professionals to ensure your safety and well-being.
