Menstrual and urinary issues?

Hello, doctor.
I would like to ask about my menstrual cycle.
I previously had a regular cycle, but I missed my period for two months and occasionally for four months.
I consulted a doctor who mentioned that being overweight could affect progesterone secretion, so it was considered normal.
Recently, my periods have returned to being regular; is this normal? Sometimes the flow is very light one month and normal the next, which makes me worry if I am approaching menopause.

Additionally, I used to have a strong urine stream, but recently I felt some discomfort in my lower abdomen (like a cramp), and it seems like my urine stream has changed.
Even when I feel a strong urge to urinate, the sensation feels different, and my urine color has been very light, even though I haven't been drinking much water.
It has returned to a more normal color now, but I am still quite concerned.

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the factors affecting the menstrual cycle are numerous, including age, environment, lifestyle, stress, emotional instability, weight changes, medications, emergency contraceptive pills, diet, polycystic ovary syndrome, uterine or cervical lesions, infections, ovulation, pregnancy, and breastfeeding, among others.
If necessary, it is advisable to consult a gynecologist for an examination.
For urinary symptoms, please have your doctor arrange for a urinalysis.
Thank you! Wishing you peace and health, Dr.
Zhang.

Understanding menstrual irregularities and urinary changes in women can be complex, especially when considering various factors such as hormonal balance, body weight, and underlying health conditions. Based on your description, it seems you have experienced fluctuations in your menstrual cycle, which can be influenced by several factors, including hormonal changes, stress, lifestyle, and body weight.

Firstly, it is important to understand that menstrual cycles can vary significantly among women and even for the same woman over time. A typical menstrual cycle ranges from 21 to 35 days, with bleeding lasting from 2 to 7 days. Irregularities, such as missed periods or variations in flow, can occur due to hormonal imbalances, which may be influenced by factors like body weight. As your doctor mentioned, being overweight can affect the secretion of progesterone, a hormone critical for regulating the menstrual cycle. This can lead to irregular periods or changes in menstrual flow, such as lighter or heavier bleeding.

Regarding your concern about approaching menopause, it is essential to note that perimenopause, the transitional phase leading up to menopause, can begin in a woman's 40s. Symptoms may include irregular periods, hot flashes, night sweats, and changes in menstrual flow. However, experiencing irregular periods does not automatically indicate that you are entering menopause. It is advisable to monitor your symptoms and discuss them with your healthcare provider, who can perform tests to assess your hormone levels and determine if you are in perimenopause.

As for your urinary changes, it is not uncommon for women to experience variations in urinary function due to hormonal fluctuations, pelvic floor muscle tone, or even stress. A decrease in urinary stream strength can sometimes be related to pelvic floor dysfunction or bladder issues. If you have experienced a sudden change in urinary function, such as a weaker stream or discomfort, it is essential to consult a healthcare professional. They can evaluate for potential causes, including urinary tract infections (UTIs), bladder conditions, or pelvic floor disorders.

The color of your urine can also provide insights into your hydration status and overall health. Light-colored urine typically indicates good hydration, while darker urine can suggest dehydration or other underlying issues. If you notice persistent changes in urine color or any discomfort while urinating, it is crucial to seek medical advice.

In summary, while fluctuations in your menstrual cycle and urinary changes can be concerning, they are often influenced by various factors, including hormonal balance and lifestyle. It is essential to maintain open communication with your healthcare provider, who can help you navigate these changes and provide appropriate guidance. Regular check-ups and monitoring your symptoms will help ensure that any potential issues are addressed promptly, allowing you to maintain your overall health and well-being.
