Urethral laceration?

Yesterday, during intercourse with my husband, it seems that an incorrect position caused his penis to slip out and forcefully hit the area of my urethral opening.
It was very painful at the moment, and afterward, I experienced swelling.
When I went to the bathroom to urinate, there was a slight stinging sensation, and when I wiped with toilet paper, there was blood.
The next morning, I wiped again and still noticed blood.
Could this be a laceration of the urethral opening? Is surgery necessary?

Hello: Generally, if there is a skin break with bleeding, it is important to first ensure cleanliness and dryness.
I have personally never encountered a situation that required surgery.
Just make sure to take good care of the wound to prevent inflammation.
If symptoms persist, please seek medical attention.
Wishing you good health.

More Info

Urethral tears can be a painful and distressing condition, often resulting from trauma or injury to the urethra, which is the tube that carries urine from the bladder to the outside of the body. In your case, it sounds like the incident during intercourse may have caused a urethral injury, leading to symptoms such as pain, swelling, and bleeding.

Symptoms of Urethral Tears
The symptoms you are experiencing—pain, swelling, and blood during urination—are indeed indicative of a potential urethral injury. Other common symptoms of urethral tears may include:
1. Pain: This can occur during urination or in the pelvic region.

2. Bleeding: Blood may be visible in the urine or at the urethral opening.

3. Difficulty urinating: This may manifest as a weak stream or an inability to urinate.

4. Swelling: Swelling around the urethra or in the genital area may be present.

5. Infection signs: Fever, chills, or increased urgency to urinate may indicate an infection.

Causes of Urethral Tears
Urethral tears can occur due to various reasons, including:
- Trauma: This can result from accidents, falls, or, as in your case, vigorous sexual activity.

- Medical procedures: Catheterization or other surgical interventions can sometimes lead to injury.

- Infections: Certain infections can weaken the urethra, making it more susceptible to tears.

Treatment Options
The treatment for a urethral tear depends on the severity of the injury. Here are some potential approaches:
1. Conservative Management: If the tear is minor, treatment may involve:
- Rest: Avoiding further irritation or trauma to the area.

- Pain Management: Over-the-counter pain relievers may help alleviate discomfort.

- Hydration: Drinking plenty of fluids can help dilute urine and reduce irritation during urination.

2. Medical Evaluation: It is crucial to seek medical attention, especially since you are experiencing ongoing bleeding. A healthcare provider may perform a physical examination and possibly imaging studies (like a retrograde urethrogram) to assess the extent of the injury.

3. Surgical Intervention: If the tear is significant or if there are complications (such as a complete rupture), surgical repair may be necessary. This would typically involve a urologist, who specializes in urinary tract issues.

4. Follow-Up Care: After treatment, follow-up appointments may be necessary to ensure proper healing and to monitor for any complications, such as infections or strictures (narrowing of the urethra).

When to Seek Immediate Care
You should seek immediate medical attention if you experience any of the following:
- Severe pain that does not improve with over-the-counter medications.

- Increased bleeding or blood in your urine that persists.

- Signs of infection, such as fever or chills.

- Difficulty urinating or inability to urinate.

Conclusion
Given your symptoms of pain, swelling, and ongoing bleeding, it is advisable to consult a healthcare professional as soon as possible. They can provide a thorough evaluation and determine the appropriate course of action based on the severity of your injury. Early intervention can help prevent complications and promote better healing outcomes. Remember, it is always better to err on the side of caution when it comes to potential injuries involving the urinary tract.
