High astigmatism?
I would like to ask the doctor if high astigmatism makes it impossible to correct vision to 1.0.
My vision is right eye 400/350 and left eye 450/325.
Can early-stage keratoconus be detected using a corneal topographer? Besides congenital reasons, what other factors can contribute to an increase in high astigmatism?
Wu Yan, 20~29 year old female. Ask Date: 2022/02/01
Dr. Liang Ce reply Ophthalmology
It is recommended that you visit an ophthalmology clinic to see if any relevant examinations are needed.
Thank you, and wish you good health!

High astigmatism can indeed be corrected to 1.0 vision (or 20/20 vision in the United States), but the degree of correction depends on several factors, including the severity of the astigmatism, the overall health of the eye, and the type of corrective measures taken.
Astigmatism occurs when the cornea (the clear front surface of the eye) or the lens inside the eye has an irregular shape. Instead of being perfectly spherical, the cornea may be more oval, causing light to focus on multiple points rather than a single point on the retina. This can lead to blurred or distorted vision at all distances.
In your case, with astigmatism measurements of 400/350 and 450/325, it is considered moderate to high astigmatism. The ability to achieve 1.0 vision will depend on how well your eyes respond to corrective lenses (glasses or contact lenses) or surgical options.
1. Corrective Lenses: Glasses or contact lenses specifically designed to correct astigmatism can significantly improve vision. Toric lenses are often used for this purpose, as they have different powers in different meridians of the lens to compensate for the uneven curvature of the cornea. Many individuals with high astigmatism can achieve 20/20 vision with the right prescription.
2. Surgical Options: For some patients, refractive surgery such as LASIK or PRK may be an option. These procedures reshape the cornea to correct the irregular curvature. However, the suitability for surgery depends on various factors, including the thickness of the cornea, the degree of astigmatism, and the presence of other eye conditions. Cone-shaped corneas, as seen in keratoconus, may complicate surgical options, but there are specialized procedures available for such cases.
3. Keratoconus: You mentioned having early-stage keratoconus. This condition can lead to progressive changes in the cornea, which may exacerbate astigmatism. Early detection using corneal topography (the corneal mapping you referred to) is crucial. This test can reveal the shape and contour of the cornea, helping to diagnose keratoconus and assess its severity.
4. Causes of Increased Astigmatism: High astigmatism can be congenital (present at birth) or develop over time due to various factors. Some potential causes include:
- Genetics: A family history of astigmatism or other refractive errors can increase the likelihood of developing high astigmatism.
- Eye Conditions: Conditions like keratoconus, as mentioned, can lead to increased astigmatism.
- Eye Surgery: Previous eye surgeries can alter the shape of the cornea and lead to astigmatism.
- Injury or Disease: Trauma to the eye or diseases affecting the cornea can also contribute to changes in astigmatism.
In conclusion, while high astigmatism can be challenging, it is often manageable with the right corrective measures. Regular eye examinations and consultations with an eye care professional are essential to monitor your vision and eye health, especially with the presence of keratoconus. If you have concerns about your vision or the progression of your astigmatism, it is advisable to discuss these with your eye doctor, who can provide personalized recommendations based on your specific situation.
