Retracting the foreskin?
Today, I retracted the foreskin down to the glans, exposing the entire head of the penis for the first time.
However, I had not retracted the foreskin before, so there is a lot of smegma present.
I noticed that touching the glans is very painful, and the smegma seems to have dried and stuck to the surface, making it difficult to clean.
I have since pulled the foreskin back over the glans.
I would like to know how to clean the smegma and whether I should retract the foreskin again in the future.

Hello: Soak with warm water, use a cotton swab to remove debris, and make sure to dry the area with a cotton swab before retracting the foreskin.
If there is significant redness or any broken skin, seek medical attention.
If the skin looks fine, remember to wash with water daily, dry the area, and gently push the foreskin back.
Wishing you good health.

More Info
When it comes to managing foreskin issues and ensuring proper hygiene, it's essential to approach the situation with care and understanding. The discomfort you're experiencing, especially with the pain upon touching the glans (the head of the penis), is not uncommon, particularly if there has been a buildup of smegma (the substance you referred to as "包皮垢"). Here’s a comprehensive guide on how to safely clean and manage these issues.
Understanding Smegma and Its Impact
Smegma is a combination of dead skin cells, oils, and moisture that can accumulate under the foreskin if it is not regularly retracted and cleaned. While it is a natural substance, excessive buildup can lead to discomfort, irritation, and even infections. The pain you're feeling when touching the glans may be due to irritation from the smegma or sensitivity from having the foreskin retracted for the first time.
Steps for Safe Cleaning
1. Gentle Retraction: If you have not been retracting your foreskin regularly, it’s important to do so gently. Avoid forcing it back if it feels painful. You should only retract it as far as it comfortably goes.
2. Warm Water Rinse: Start by rinsing the area with warm water. This can help soften the smegma, making it easier to clean. Avoid using soap initially, as it can irritate the sensitive skin of the glans.
3. Use a Soft Cloth: After rinsing, take a soft, clean cloth or a cotton ball and gently wipe the area. If the smegma is particularly stubborn, you can soak the cloth in warm water and apply it to the area for a few minutes to help loosen it.
4. Mild Soap (if necessary): If the smegma does not come off with water alone, you can use a mild, unscented soap. Be cautious to avoid any harsh chemicals or fragrances that could irritate the skin. Gently wash the area, ensuring that you rinse thoroughly to remove all soap residue.
5. Drying: After cleaning, gently pat the area dry with a clean towel. Avoid rubbing, as this can cause further irritation.
6. Moisturizing: If the glans feels dry or irritated after cleaning, consider applying a small amount of a gentle, fragrance-free moisturizer. This can help soothe the skin.
Managing Future Hygiene
- Regular Cleaning: Make it a habit to gently retract the foreskin and clean the area regularly (ideally daily) to prevent the buildup of smegma. This will help maintain hygiene and reduce the risk of irritation or infection.
- Monitor for Symptoms: Keep an eye out for any signs of infection, such as increased redness, swelling, discharge, or persistent pain. If you notice any of these symptoms, it’s important to consult a healthcare professional.
- Consult a Doctor: If you continue to experience pain or if the smegma buildup is difficult to manage, consider seeking advice from a urologist. They can provide guidance tailored to your specific situation and may recommend further treatments if necessary.
Conclusion
Maintaining proper hygiene of the foreskin is crucial for overall genital health. By following the steps outlined above, you can effectively clean the area and manage any discomfort. Remember, if you have any concerns or if symptoms persist, do not hesitate to reach out to a healthcare professional for assistance. They can provide you with the best advice and treatment options tailored to your needs.
