Scrotal itching

In the past month, I have experienced significant itching on the sides of my scrotum and genital area, particularly at the base.
The itching worsens with pressure or touch, leading to several scratches on the skin.
Applying petroleum jelly after showering at night helps alleviate the itching while sleeping, but during the day or after returning from work, the itching persists.
Could this be eczema or something else? What are some simple solutions? Should I reduce cleaning or do something else? There are no noticeable changes in appearance or color, except for scratches on the front of the scrotum and two pinpoint abrasions on the left side of the foreskin.
Should I consult a dermatologist or a urologist for this condition?

Yi, 40~49 year old female. Ask Date: 2022/02/20

More Info

Scrotal itching can be a distressing symptom and may arise from various causes. Understanding the underlying reasons for this discomfort is crucial for effective management and treatment.

Causes of Scrotal Itching
1. Dermatitis: This includes conditions like eczema or contact dermatitis, which can occur due to irritants or allergens. If you've recently changed soaps, laundry detergents, or personal care products, these could be culprits.

2. Fungal Infections: Tinea cruris, commonly known as jock itch, is a fungal infection that can cause intense itching in the groin area, including the scrotum. It thrives in warm, moist environments.

3. Bacterial Infections: Bacterial infections can also lead to itching and irritation. Folliculitis, an infection of hair follicles, can cause localized itching.

4. Parasitic Infections: Conditions like scabies or pubic lice can cause severe itching and discomfort in the genital area.

5. Skin Conditions: Other skin conditions, such as psoriasis or psoriasis-like lesions, can also manifest as itching in the scrotal area.

6. Hygiene Issues: Inadequate hygiene can lead to the accumulation of sweat and bacteria, causing irritation and itching. Conversely, excessive washing can strip the skin of natural oils, leading to dryness and irritation.

7. Allergic Reactions: Allergies to certain materials, such as latex in condoms or certain fabrics, can also cause itching.

Remedies for Scrotal Itching
1. Maintain Hygiene: Regular washing with mild soap and water can help keep the area clean. Ensure to dry the area thoroughly, as moisture can exacerbate fungal infections.

2. Use Moisturizers: Applying a gentle, fragrance-free moisturizer can help soothe dry skin. While you mentioned using Vaseline, consider using a lighter moisturizer that won’t clog pores.

3. Avoid Irritants: Identify and avoid any potential irritants, such as harsh soaps, lotions, or fabrics that may be causing the reaction.

4. Wear Loose Clothing: Opt for breathable, loose-fitting underwear made from natural fibers like cotton to reduce moisture and friction.

5. Over-the-Counter Treatments: Antifungal creams can be effective if a fungal infection is suspected. Hydrocortisone cream may help reduce inflammation and itching.

6. Cold Compress: Applying a cold compress to the affected area can provide temporary relief from itching.

When to Seek Help
If the itching persists despite home remedies, or if you notice any of the following, it’s important to seek medical attention:
- Severe or worsening symptoms: If the itching becomes unbearable or leads to significant discomfort.

- Signs of infection: If you notice redness, swelling, discharge, or increased pain.

- Persistent lesions: If you develop new spots, rashes, or lesions that do not improve.

- Systemic symptoms: If you experience fever, chills, or other systemic symptoms.

Which Specialist to See
Given your symptoms, it would be appropriate to consult a dermatologist first, as they specialize in skin conditions and can evaluate for dermatitis, fungal infections, or other skin-related issues. If the dermatologist rules out skin conditions and suspects a urological issue, they may refer you to a urologist for further evaluation.

In summary, scrotal itching can stem from various causes, and understanding these can help in managing the symptoms effectively. Maintaining good hygiene, avoiding irritants, and using appropriate treatments can provide relief. However, persistent or severe symptoms warrant a visit to a healthcare professional for a thorough evaluation and tailored treatment plan.
