Inner thigh swelling and pain?

The family member has been experiencing thigh swelling and pain for the past two to three months.
Initially, the discomfort began while riding a motorcycle, where prolonged riding would cause a tingling sensation in the inner thigh.
The most intense pain occurred just before the Lunar New Year when using the restroom; the right thigh experienced sharp pain that made it difficult to sit.
Upon examination, there is a noticeable lump on the inner thigh, which feels warm and tight.
While sitting or lying down does not cause much pain, pressing on the swollen area and walking results in significant pain.
The most similar case I found was from 13 years ago, but it did not specify which specialty to consult.
I would like to take my mother to a rehabilitation clinic, but she is reluctant, believing that it would only involve heat therapy.
I hope the doctor can provide some guidance.
Thank you.

Hello: Based on your description: ........
the inner side feels like there is a lump, and it feels warm and tight; sitting or lying down doesn't hurt much, but pressing on the lump makes walking very painful........
It is suggested to first rule out a lump in the thigh (lymphadenopathy or other conditions...).
You can first consult a physician who performs diagnostic soft tissue ultrasound (usually a rehabilitation medicine physician).
If the lump is ruled out, you can then see a neurosurgeon for further evaluation.
Best wishes to you // Neurosurgeon Dr.

Inner thigh pain can be a complex issue, often resulting from a variety of causes ranging from musculoskeletal problems to nerve issues. Given the symptoms you've described, including persistent pain, a sensation of numbness, and the presence of a lump, it’s essential to consider several potential underlying conditions.

Possible Causes of Inner Thigh Pain
1. Muscle Strain or Injury: Overuse or strain from activities such as prolonged motorcycle riding can lead to muscle fatigue or injury. This is particularly true if the muscles are not accustomed to such activity or if proper posture is not maintained.

2. Nerve Compression or Entrapment: Conditions such as meralgia paresthetica, where the lateral femoral cutaneous nerve is compressed, can cause numbness and pain in the thigh. This can occur due to prolonged sitting or tight clothing.

3. Hernia: An inguinal hernia can present as a lump in the groin area and may cause pain that radiates into the thigh. This is especially relevant if the lump is palpable and associated with discomfort.

4. Lymphadenopathy: Swollen lymph nodes in the groin can cause pain and a feeling of tightness. This can be due to infections or other systemic issues.

5. Tendonitis or Bursitis: Inflammation of the tendons or bursae around the hip joint can lead to pain that may radiate down the thigh.

6. Deep Vein Thrombosis (DVT): Although less common, a blood clot in the deep veins of the leg can cause swelling, pain, and a feeling of warmth in the affected area. This is a medical emergency and requires immediate attention.

Treatment Options
1. Rest and Activity Modification: Reducing activities that exacerbate the pain is crucial. If motorcycle riding is a contributing factor, consider taking breaks or adjusting your riding posture.

2. Physical Therapy: Engaging in physical therapy can help strengthen the muscles around the thigh and improve flexibility. A physical therapist can provide tailored exercises and modalities to alleviate pain.

3. Heat and Cold Therapy: Applying heat can help relax and soothe tight muscles, while cold therapy can reduce inflammation and numb sharp pain.

4. Medication: Over-the-counter nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen can help reduce pain and inflammation. If the pain persists, a healthcare provider may prescribe stronger medications.

5. Diagnostic Imaging: If the pain continues or worsens, imaging studies such as an ultrasound or MRI may be necessary to assess for structural issues, such as tears or hernias.

6. Consultation with Specialists: Depending on the findings, it may be beneficial to consult with an orthopedic specialist, neurologist, or a physical medicine and rehabilitation physician for a comprehensive evaluation.

Conclusion
Given the duration and nature of the symptoms, it’s important to seek medical attention to rule out serious conditions such as DVT or hernia. While your family member may be hesitant to visit a rehabilitation clinic, a thorough evaluation by a healthcare professional is essential to determine the exact cause of the pain and to develop an effective treatment plan. Encourage them to consider the potential benefits of professional assessment and treatment, as early intervention can lead to better outcomes and relief from discomfort.
