Itching more than a month after circumcision?
Hello, doctor.
I underwent a circumcision procedure at the end of January this year.
The process went smoothly, and after the stitches fell out, the doctor examined the wound and said it was healing well.
However, I occasionally experience itching around the frenulum and the scar area.
There is some swelling in the frenulum area that feels somewhat like the initial edema, but not exactly the same.
The doctor previously prescribed Meikang for me to apply, but it seems that the issue has not resolved.
I'm unsure if this is due to the wound still being in the healing phase or if it’s because there is too much foreskin left, which could be causing inflammation.
When I am not erect, I notice a small ring of skin accumulating around the coronal sulcus.
The doctor mentioned that this should be left for when I am erect, but I am a bit worried.
If there is inflammation after circumcision, could it mean I might need a second surgery? Just thinking about it is a bit discouraging.
Thank you for taking the time to respond, doctor.

Hello: After surgery, it is important to keep the area clean and dry.
If the foreskin is clean, it is less likely to become inflamed.
Some swelling and itching are normal after the procedure, as long as there is no redness, there is no cause for concern.
Wishing you good health.

Persistent itching after circumcision can be a common concern for many patients, and it can arise from various causes. Understanding these causes and potential solutions is essential for effective management.
Causes of Itching After Circumcision
1. Healing Process: After circumcision, the skin around the surgical site undergoes a healing process. This can involve inflammation, which may lead to itching. The sensation is often due to the regeneration of nerve endings and the formation of scar tissue. It is not uncommon for the healing skin to feel itchy as it recovers.
2. Scar Tissue Formation: As the incision heals, scar tissue can form, which may be more sensitive than the surrounding skin. This sensitivity can lead to persistent itching, especially if the scar tissue is raised or irregular.
3. Moisture and Hygiene: The area around the circumcision site can retain moisture, especially if proper hygiene is not maintained. This can create an environment conducive to irritation or even infection, leading to itching. It is crucial to keep the area clean and dry.
4. Allergic Reactions: Sometimes, the use of topical ointments or creams (like the one prescribed, Mecon) can cause allergic reactions or irritation, leading to itching. If you have recently started using a new product, consider whether it could be contributing to your symptoms.
5. Infection: Although you mentioned that the doctor assessed the healing and found no issues, it is still possible for a mild infection to develop post-surgery. Signs of infection can include increased redness, swelling, warmth, or discharge from the site, in addition to itching.
6. Excess Skin: If the circumcision left excess skin, it could lead to irritation during movement or when clothing rubs against the area. This can also contribute to itching and discomfort.
Solutions and Recommendations
1. Follow-Up with Your Doctor: Since you are experiencing persistent itching, it is advisable to follow up with your healthcare provider. They can assess the area for any signs of infection or complications and provide tailored advice based on your specific situation.
2. Maintain Hygiene: Ensure that you are keeping the area clean and dry. Gently wash the area with mild soap and water, and pat it dry. Avoid using harsh soaps or products that could irritate the skin.
3. Topical Treatments: If your doctor approves, consider using over-the-counter hydrocortisone cream to reduce itching and inflammation. However, it is essential to consult your doctor before applying any new treatment.
4. Avoid Irritants: Be mindful of any products you apply to the area, including lotions, creams, or even laundry detergents that may come into contact with your genital area. Switching to hypoallergenic products may help.
5. Monitor for Other Symptoms: Keep an eye on any additional symptoms that may arise, such as increased pain, swelling, or discharge. If these occur, seek medical attention promptly.
6. Patience with Healing: Understand that the healing process can take time, and some itching may be a normal part of recovery. However, if the itching persists beyond a reasonable period or worsens, further evaluation may be necessary.
7. Consider a Second Opinion: If you remain concerned about the possibility of needing a second surgery or if your symptoms do not improve, seeking a second opinion from another urologist or specialist may provide additional insights and reassurance.
In summary, persistent itching after circumcision can stem from various factors, including the healing process, scar tissue formation, hygiene issues, allergic reactions, or even mild infections. It is crucial to maintain good hygiene, monitor your symptoms, and consult with your healthcare provider for appropriate management. With proper care and attention, most patients find relief from their symptoms and continue to heal effectively.
