Bleeding after sexual intercourse?

Hello Dr.
Tsao, I have recently experienced some bleeding during intercourse with my husband, although it does not happen every time.
The amount of bleeding is minimal and stops shortly after.
The color of the blood is bright red.
My usual discharge is either clear or white.
I have seen a doctor and had a vaginal ultrasound, which came back normal.
I have never experienced this issue before, and I gave birth two years ago.
I would appreciate your advice.
Thank you.

Post-coital bleeding, or bleeding that occurs after sexual intercourse, can be concerning and may arise from various causes. Given your situation, where the bleeding is intermittent and occurs after sexual activity, it's essential to consider several potential reasons for this symptom.

1. Cervical Erosion or Ectropion: This condition occurs when the cells from the inside of the cervical canal grow on the outside of the cervix. It can be sensitive and may bleed after intercourse. This is often benign but should be monitored.

2. Vaginal Dryness or Trauma: Insufficient lubrication during intercourse can lead to friction and minor injuries to the vaginal walls, resulting in bleeding. This can be more common in women who are breastfeeding or have recently given birth, as hormonal changes can affect vaginal moisture.

3. Infections: Conditions such as cervicitis (inflammation of the cervix) or vaginitis (inflammation of the vagina) can cause bleeding. These infections may also present with unusual discharge or discomfort.

4. Polyps or Fibroids: Benign growths on the cervix or within the uterus can cause bleeding, especially during or after intercourse. These are generally non-cancerous but should be evaluated by a healthcare provider.

5. Hormonal Changes: Fluctuations in hormone levels, particularly around the menstrual cycle, can lead to unexpected bleeding. This is often benign but should be discussed with a healthcare provider if it persists.

6. Malignancy: Although less common, post-coital bleeding can sometimes indicate more serious conditions, such as cervical or endometrial cancer. This is particularly a concern if the bleeding is accompanied by other symptoms, such as unusual discharge or pelvic pain.

Since you have already undergone a vaginal ultrasound and received a normal result, this is reassuring. However, if the bleeding continues or worsens, or if you experience additional symptoms such as pain, changes in discharge, or any other concerning signs, it is crucial to seek further medical evaluation.
In terms of when to seek medical advice, consider the following guidelines:
- If the bleeding persists for more than a few cycles or becomes more frequent.

- If you experience significant pain or discomfort during or after intercourse.

- If there are changes in your menstrual cycle or unusual discharge.

- If you have any concerns about the nature of the bleeding or if it is accompanied by other symptoms.

In the meantime, you might consider using a water-based lubricant during intercourse to reduce friction and potential trauma to the vaginal walls. This can help alleviate symptoms if dryness is a contributing factor.

Lastly, maintaining open communication with your healthcare provider is essential. If you feel that your concerns are not being adequately addressed, seeking a second opinion can provide additional reassurance and clarity regarding your health.
