Dripping white phenomenon
Hello, doctor.
Occasionally, when I strain to have a bowel movement while sitting on the toilet and then urinate, I notice a white dripping phenomenon in the toilet.
However, there is no residual dripping or unusual signs on the glans.
The toilet contains a thick fluid that resembles prostatic fluid.
This only occurs when I strain to have a bowel movement; if I do not strain, it does not happen.
I also frequently experience this when I strain to urinate while sitting, but there are no issues with prostatic fluid when I urinate while standing.
In the morning and before showering at night, I check my underwear and find no residual fluid.
After researching, I found that some suggest it could be prostatitis, while others say that straining can compress the prostate, making this situation normal.
I do not experience frequent urination, urgency, nocturia, or painful urination.
I would appreciate your clarification on this matter.
Thank you.

Hello: If this has occurred recently, it could be urethral discharge.
If there has been sexual activity, a hospital examination is recommended, and at least a urinalysis should be performed.
Symptoms related to prostate issues can vary widely, making it difficult to determine the exact cause.
Wishing you good health.

The phenomenon you are describing, often referred to as "dripping white" or the presence of a milky or cloudy discharge in the toilet after urination, can be concerning for many individuals. It is important to understand the potential causes and implications of this symptom, especially in the context of urological health.
Firstly, the appearance of a white or cloudy substance in the urine can be attributed to several factors. One common cause is the presence of prostatic fluid, which may be released during urination, especially if there is any straining involved. The prostate gland produces a fluid that is a component of semen, and during activities that involve pressure on the pelvic region, such as straining to defecate, this fluid can sometimes be expelled. This is generally not a cause for concern if it occurs infrequently and is not accompanied by other symptoms.
However, if you notice this phenomenon consistently, it may warrant further investigation. While you mentioned that you do not experience other symptoms such as frequent urination, urgency, or pain during urination, it is still advisable to consider a few possibilities:
1. Prostatitis: Inflammation of the prostate gland can lead to various symptoms, including changes in urinary output. While you do not have typical symptoms of prostatitis, it can sometimes present with atypical signs.
2. Urinary Tract Infection (UTI): Although you do not report symptoms commonly associated with UTIs, such as burning during urination or increased frequency, it is still possible to have a UTI that presents differently. A urinalysis can help rule this out.
3. Dehydration or Concentrated Urine: If you are not adequately hydrated, your urine may appear more concentrated and could potentially have a cloudy appearance. Ensuring adequate fluid intake can help clarify urine.
4. Dietary Factors: Certain foods and supplements can affect urine color and clarity. For example, high protein diets or certain vitamins can lead to changes in urine appearance.
5. Sexual Activity: If you have engaged in sexual activity recently, the presence of seminal fluid can also contribute to the appearance of a white substance in the toilet.
Given that you do not have other concerning symptoms, it may be reasonable to monitor the situation for any changes. However, if the "dripping white" phenomenon persists or if you develop additional symptoms, it would be prudent to consult a urologist for a thorough evaluation. This may include a physical examination, urinalysis, and possibly imaging studies to assess the prostate and urinary tract.
In conclusion, while the occasional presence of a white substance in the toilet after urination may not be alarming, it is essential to remain vigilant about your urological health. Regular check-ups and open communication with your healthcare provider can help ensure that any potential issues are addressed promptly. If you have any further concerns or if the situation changes, do not hesitate to seek medical advice.
