Chronic prostatitis with perineal twitching sensation?


Hello Doctor, I am 28 years old.
Last year, I sought medical attention due to occasional twitching sensations in the perineum and discomfort in the penis.
I was later diagnosed with chronic prostatitis, but long-term medication and shockwave therapy have been ineffective.
I have been under treatment for almost a year now, and the psychological burden of this illness is significant.

Currently, my symptoms include a twitching sensation in the perineum, which can be rapid at times or occur intermittently.
I also experience occasional mild pain at the base of the penis on both sides (sometimes the pain extends slightly into the penis during an erection), and the scrotum often feels tense.
In recent months, I have been unable to sit or stand for long periods, as I feel discomfort in the nerves from my buttocks and perineum to the inner thighs (I have a slim build with little fat in my buttocks).

I would like to know if this is solely due to chronic prostatitis, or if there are other factors to consider.
What further examinations can be conducted? Thank you.

Currently, I do daily stretching exercises for my glutes, but the results are limited.
