Nipple discharge, ductoscopy?

Last October, during a self-examination of my breasts, I noticed discharge from my right nipple when pressed.
Within just two weeks, the discharge changed from amber to blood.
After visiting a second hospital, I underwent blood tests, ductography, and ultrasound in January this year, which confirmed a diagnosis of a mammary duct tumor.
I had surgery to remove the tumor in early February.
However, two weeks after the surgery, when the dressing was removed, I experienced significant itching of the nipple and breast skin, leading to frequent scratching.
A month post-surgery, during another self-examination, I noticed bleeding again.
During the pre-operative consultations, the doctor mentioned that removing the tumor does not guarantee it won't recur.
The doctor also suggested that I could undergo ductoscopy and informed me that Changhua Christian Hospital has this equipment.
However, since I live in Taipei, I have difficulty finding relevant medical information or hospitals with ductoscopy equipment online.
Now that I am experiencing bleeding again and cannot undergo imaging in the short term, I would like to inquire about which hospitals in Taiwan currently offer ductoscopy or have the necessary equipment so that I can arrange an appointment as soon as possible.
Thank you.

To my knowledge, only Changhua Christian Hospital currently offers this examination; other hospitals may require you to check online for availability.

Understanding nipple discharge can be a complex issue, particularly when it involves conditions like ductal papilloma, which you have already been diagnosed with. Ductoscopy is a minimally invasive procedure that allows for direct visualization of the ducts within the breast, and it can be particularly useful in cases of abnormal nipple discharge. In Taiwan, several medical facilities are equipped to perform ductoscopy, but finding the right one can be challenging, especially if you are located in Taipei.

Firstly, it’s important to understand that nipple discharge can arise from various causes, including benign conditions like ductal papilloma, infections, or more serious conditions such as breast cancer. The fact that you have experienced a change in the nature of your discharge—from amber to blood—warrants careful monitoring and follow-up. Your previous surgery for ductal papilloma was a step in the right direction, but as your doctor indicated, the removal of the papilloma does not guarantee that new growths will not develop.

Ductoscopy can be particularly beneficial in your case. It allows for the visualization of the ductal system and can help identify any new lesions or abnormalities that may be causing the discharge. During the procedure, a thin, flexible tube with a camera is inserted into the duct, allowing the physician to see inside and potentially take biopsies if necessary. This can provide valuable information that imaging studies like mammograms or ultrasounds might not reveal.

In Taiwan, several hospitals and medical centers are known to have the capability to perform ductoscopy. While I cannot provide an exhaustive list, you may want to consider contacting the following types of facilities:
1. University Hospitals: Major university hospitals often have specialized breast centers. For example, National Taiwan University Hospital and Taipei Medical University Hospital are known for their comprehensive breast care services.

2. Breast Clinics: Some clinics specialize in breast health and may offer ductoscopy as part of their services. It’s advisable to check if they have the necessary equipment and expertise.

3. Private Hospitals: Some private hospitals in Taipei may also offer ductoscopy. Hospitals like Shin Kong Wu Ho-Su Memorial Hospital and Cathay General Hospital are known for their advanced medical technologies.

4. Referral from Your Current Physician: Since you have already been in contact with a physician regarding your condition, it may be beneficial to ask them for a referral to a facility that offers ductoscopy. They may have partnerships or knowledge of specific hospitals that can accommodate your needs.

Regarding your symptoms of itching and the recurrence of bleeding, it’s crucial to communicate these issues with your healthcare provider. They may recommend further evaluation or treatment options to address these symptoms, especially since they could indicate complications or the need for additional intervention.

In conclusion, while ductoscopy is a valuable tool for diagnosing and managing conditions related to nipple discharge, it’s essential to find a facility that can provide this service. Reach out to major hospitals in your area, consult with your current healthcare provider for recommendations, and ensure that you continue to monitor your symptoms closely. Your health and peace of mind are paramount, and timely intervention can make a significant difference in your care.
