COVID-19 vaccine
I received my first and second doses of the AstraZeneca vaccine, and I am now preparing to get my third dose.
I previously experienced redness and itching after receiving a contrast agent during a CT scan, but I managed it by drinking water without taking any medication, and the symptoms resolved after about 3 to 4 hours.
Can I mix my third dose with either the Pfizer-BioNTech or Moderna vaccine?

Dr. Lin Zhebin reply Family Medicine
Hello: Since you experience redness and itching after receiving the contrast agent, there is a suspicion that you may have an allergic reaction to it.
Currently, it is suspected that an allergy to the contrast agent may increase the risk of allergic reactions to mRNA vaccines (Moderna, BNT), but there is a lack of large-scale studies to confirm this.
Therefore, individuals with a contrast agent allergy can consider receiving non-mRNA vaccines, but it does not mean that mRNA vaccines are completely off-limits.
Given your allergy to the contrast agent, it is not an immediate anaphylactic shock, so if you really want to receive an mRNA vaccine, it is still possible.
However, in addition to being observed for 30 minutes after vaccination, you should be prepared for potential allergic reactions such as itching and redness.
Additionally, since the third dose of the vaccine with AstraZeneca is not recommended, if you choose a non-mRNA vaccine, the only option left is the High-End vaccine.

When considering the mixing of COVID-19 vaccines, particularly after experiencing an allergic reaction to a contrast dye, it is essential to approach the situation with caution and informed guidance. You mentioned that you received the AstraZeneca (AZ) vaccine for your first two doses and are now contemplating a third dose with either the Pfizer-BioNTech (BNT) or Moderna mRNA vaccines.
First, it is important to understand the nature of your previous allergic reaction. The reaction you described—redness and itching after receiving a contrast dye—suggests a possible allergic response, but it is crucial to note that this was not an immediate severe allergic reaction (such as anaphylaxis). Immediate allergic reactions typically occur within minutes to a few hours after exposure, while your symptoms resolved after a few hours without medical intervention. This distinction is important because it may influence the risk assessment for receiving an mRNA vaccine.
Current guidelines suggest that individuals who have had a non-severe allergic reaction to a substance (like a contrast dye) can generally receive mRNA vaccines, including BNT and Moderna. However, there is a noted concern regarding individuals with a history of allergic reactions to certain substances, as they may have a higher risk of experiencing allergic reactions to mRNA vaccines. This is particularly relevant for those who have had severe allergic reactions in the past, but your case appears to be less severe.
The Centers for Disease Control and Prevention (CDC) and other health authorities recommend that individuals with a history of allergies should be monitored for 30 minutes after receiving an mRNA vaccine. This is to ensure that if any allergic reaction occurs, it can be promptly addressed. Given your previous experience with the contrast dye, it would be prudent to inform the healthcare provider administering the vaccine about your history, so they can take appropriate precautions.
In terms of vaccine efficacy, mixing vaccines has been shown to be safe and effective in various studies. For instance, research has indicated that mixing different types of COVID-19 vaccines can enhance the immune response. The combination of AZ with mRNA vaccines like BNT or Moderna has been studied and is generally considered safe.
As for your specific situation, if you decide to proceed with either BNT or Moderna for your third dose, ensure that you are in a setting where medical assistance is readily available, should you experience any adverse reactions. Additionally, it is advisable to consult with your healthcare provider for personalized medical advice, especially considering your previous allergic reaction. They can help assess your individual risk factors and provide guidance tailored to your health history.
In summary, while it is generally safe to mix COVID-19 vaccines after a non-severe allergic reaction to contrast dye, it is essential to take precautions and consult with a healthcare professional. Monitoring for any potential allergic reactions post-vaccination is crucial, and being in a medical environment can provide peace of mind. Your health and safety should always be the priority when making vaccination decisions.
