Vaccination for Ankylosing Spondylitis
Hello Doctor, I am a patient with ankylosing spondylitis.
I would like to ask you: 1) If the COVID-19 vaccine has exacerbated my pre-existing stiffness, is it possible that this exacerbation could last a long time (for example, 1-2 years, or even longer, possibly a lifetime)? 2) If it is not likely to last long and will return to the original condition, is this related to the weakening of the antibodies produced by the vaccine? In other words, does the exacerbated condition disappear because the antibodies weaken? If it is not related to the weakening of antibodies, what other reasons could explain why the exacerbated condition would not last long? Thank you, Doctor.

Hello, thank you for your question.
Here are my responses:
1) If the COVID-19 vaccine exacerbates a pre-existing rigid condition, is it possible that this exacerbation could last for a long time (for example, 1-2 years, or even longer, potentially a lifetime)? --> Currently, since the vaccine has only been in use for nearly two years, we cannot answer this question definitively.
2) If the exacerbation does not last long and returns to the original condition, could this be related to the waning of antibodies produced by the vaccine? In other words, does the worsening condition disappear because the antibodies have diminished? If it is not related to the waning of antibodies, what other reasons could explain why the exacerbated condition does not persist for a long time? --> There is currently no research to answer this question either.
I believe it is likely unrelated to antibody waning.
When the immune system experiences a temporary abnormality due to vaccination, it often gradually recovers, which is usually a result of the immune system self-regulating.
In other words, if the immune system was relatively normal to begin with, it can overcome a temporary immune dysfunction and return to normal.
However, if the immune function was already abnormal, it may worsen after vaccination and lack the ability to self-regulate and recover, potentially leading to prolonged recovery times or the emergence of new immune diseases.
Sincerely, the Allergy, Immunology, and Rheumatology Department, Changhua Hospital, Ministry of Health and Welfare.

The impact of COVID-19 vaccination on individuals with ankylosing spondylitis (AS) is a topic of growing interest, particularly as we continue to navigate the complexities of the pandemic and its effects on various health conditions. As a patient with AS, your concerns about the potential exacerbation of symptoms following vaccination are valid and warrant careful consideration.
1. Duration of Symptom Exacerbation Post-Vaccination: It is not uncommon for patients with autoimmune conditions, such as ankylosing spondylitis, to experience temporary flares in their symptoms following vaccination. This can be attributed to the immune response triggered by the vaccine, which may lead to increased inflammation in some individuals. However, the exacerbation of symptoms is generally expected to be transient. Most studies and clinical observations suggest that any increase in symptoms post-vaccination tends to resolve within a few weeks to a few months. Long-term exacerbation lasting one to two years or more is not typical. If symptoms do persist for an extended period, it may be indicative of other underlying issues or a need for reassessment of the management plan for AS.
2. Relationship Between Antibody Levels and Symptom Resolution: The resolution of exacerbated symptoms following vaccination is indeed related to the immune response, including the production of antibodies. As the immune system gradually returns to its baseline state after the initial response to the vaccine, the inflammatory processes that may have been temporarily heightened can subside. This does not necessarily mean that the symptoms are solely linked to antibody levels; rather, it reflects the dynamic nature of the immune system. Factors such as the individual’s overall health, the presence of other comorbidities, and the specific characteristics of their AS can all influence how they respond to vaccination.
3. Other Factors Influencing Symptom Duration: Beyond antibody levels, several other factors can contribute to the duration and severity of symptom exacerbation. These include:
- Individual Variation: Each person's immune system reacts differently to vaccines. Genetic predispositions, previous experiences with vaccinations, and the current state of the disease can all play a role.
- Underlying Inflammation: If a patient has ongoing inflammation due to AS, the vaccine may exacerbate these symptoms temporarily. However, this is usually a short-term effect.
- Psychological Factors: Stress and anxiety related to the pandemic or health concerns can also influence symptom perception and severity. Managing stress through relaxation techniques or counseling may help alleviate some of the exacerbated symptoms.
4. Recommendations for Patients with AS: If you experience a flare-up of symptoms following vaccination, it is essential to communicate with your healthcare provider. They can help determine whether the exacerbation is related to the vaccine or if other factors are at play. Adjustments to your treatment plan, including anti-inflammatory medications or physical therapy, may be necessary to manage symptoms effectively.
In conclusion, while it is possible for COVID-19 vaccination to temporarily exacerbate symptoms of ankylosing spondylitis, these effects are generally short-lived. The immune response, including antibody production, plays a significant role in this process. If you have concerns about your symptoms or the impact of vaccination on your condition, it is crucial to discuss these with your healthcare provider for personalized guidance and management strategies.
