Urethritis with frequent urination?

Hello Doctor, I have recently been experiencing some discomfort and bloating in my lower abdomen, along with frequent urination.
Given my previous experience with a urinary tract infection, I promptly went to the hospital for an examination.
The results confirmed that I have urethritis.
However, I am unsure why, after taking medication for 10 days, I am still not feeling better.
The bloating and pain during urination have subsided, but the issue of frequent urination persists.
Sometimes, I feel the urge to urinate immediately after just having gone.
During this infection, I have also been having trouble sleeping at night due to the constant urge to use the bathroom, which is quite distressing...
I have been drinking plenty of water and taking cranberry probiotics.
Is it normal for urethritis to not improve after 10 days of treatment? (I have no sexual experience.)

Dr. Sun Hongshun reply Urology

Hello: As you mentioned, you have been taking good care of yourself, but I am genuinely concerned that your condition has persisted for so long.
How did you determine that your urinary tract infection has not resolved? Did you have a follow-up urine test? If not, please note that frequent urination and urinary tract infections do not necessarily correlate.
I recommend that you schedule another appointment, where you can have a urine test as well as a non-invasive ultrasound examination of the urinary system (which you need not worry about).
Wishing you a speedy recovery and all the best.

Persistent urinary frequency after treatment for urethritis can be a frustrating and concerning issue, especially when you have already completed a course of antibiotics. It is important to understand that while antibiotics are effective in treating bacterial infections, they may not always resolve all symptoms associated with urinary tract infections (UTIs) or urethritis.
In your case, you mentioned that you have been on antibiotics for 10 days, and while the pain and discomfort have subsided, the frequent urge to urinate remains. This situation can arise for several reasons:
1. Residual Inflammation: After an infection, the urinary tract can remain inflamed for some time. This inflammation can lead to increased sensitivity and a persistent urge to urinate, even after the infection has been cleared. The bladder may still be reacting as if it is under threat, which can cause frequent urges to void.

2. Bladder Overactivity: Some individuals may develop a condition known as bladder overactivity or overactive bladder (OAB) following a UTI. This condition is characterized by a sudden, uncontrollable urge to urinate, often accompanied by increased frequency of urination. It can occur even in the absence of an infection.

3. Incomplete Resolution of Infection: In some cases, the initial antibiotic treatment may not fully eradicate the infection, especially if the bacteria are resistant to the prescribed antibiotic. This can lead to lingering symptoms. A follow-up urine culture may be necessary to determine if the infection persists.

4. Psychological Factors: Anxiety and stress related to previous urinary tract infections can also contribute to the sensation of urgency and frequency. The fear of experiencing pain or discomfort can create a cycle of anxiety that exacerbates the symptoms.

5. Other Underlying Conditions: Conditions such as interstitial cystitis, pelvic floor dysfunction, or even gynecological issues can mimic UTI symptoms and lead to persistent urinary frequency. It is essential to rule out these possibilities if symptoms persist.

Given your situation, here are some recommendations:
- Follow-Up with Your Healthcare Provider: Since you have been on antibiotics for 10 days without complete resolution of symptoms, it is advisable to return to your healthcare provider. They may recommend a urine culture to check for any residual infection or resistance to the antibiotic you were prescribed.

- Consider Bladder Training: If bladder overactivity is suspected, bladder training techniques can help. This involves gradually increasing the time between urinations to help retrain your bladder.

- Hydration and Diet: Continue to stay well-hydrated, as this can help flush out any lingering bacteria. However, be mindful of irritants such as caffeine, alcohol, and spicy foods, which can exacerbate bladder symptoms.

- Cranberry Products: While cranberry juice and supplements may help prevent UTIs, their effectiveness in treating existing symptoms is limited. However, they are generally safe to consume and may provide some benefit.

- Pelvic Floor Exercises: Strengthening the pelvic floor muscles through Kegel exercises can help improve bladder control and reduce urgency.

- Monitor Symptoms: Keep a diary of your symptoms, noting the frequency of urination, any associated pain, and any other relevant factors. This information can be helpful for your healthcare provider in determining the next steps.

In conclusion, while it is not uncommon to experience persistent urinary frequency after treatment for urethritis, it is essential to address this issue with your healthcare provider to rule out any underlying conditions and explore appropriate treatment options. Your comfort and well-being are paramount, and there are various strategies that can help alleviate your symptoms.
