Deep itching at the surgical wound site?
Ten years ago, a benign lipoma was excised from the web space of the hand, but even now, the deeper area (not the skin surface) at the surgical site occasionally itches.
What could be the cause of this? What methods can be used to relieve the itching? It is really bothersome.

Dr. Li Zhonghuai reply Surgery
Hello! It is important to consider whether there are any non-absorbable sutures.
If the issue is superficial, it may be a keloid.
If there are no problems, applying ice packs will suffice.

More Info
Deep itching after surgery, particularly in the area where a benign lipoma was removed, can be a frustrating and uncomfortable experience. This sensation can be attributed to several factors related to the healing process, nerve regeneration, and changes in the skin and underlying tissues.
Causes of Deep Itching
1. Nerve Regeneration: After surgery, the nerves in the area may be damaged or disrupted. As these nerves heal and regenerate, they can send abnormal signals to the brain, which may be interpreted as itching. This is particularly common in areas where the skin has been cut or manipulated.
2. Scar Tissue Formation: The body forms scar tissue as part of the healing process. This scar tissue can be denser and less elastic than normal tissue, which may lead to sensations of tightness or itching. The deeper layers of the skin and underlying tissues can be affected, leading to a sensation that feels deeper than just surface itching.
3. Dry Skin: Post-surgical areas can often become dry, especially if they are not moisturized properly. Dry skin can lead to itching, and this can be exacerbated by environmental factors or the use of certain medications.
4. Histamine Release: The surgical process can trigger an inflammatory response, leading to the release of histamines in the body. Histamines are chemicals that can cause itching and are part of the body's immune response to injury.
5. Allergic Reactions: Sometimes, the materials used during surgery (like sutures or adhesives) can cause localized allergic reactions, leading to itching and discomfort.
Relief Strategies
1. Moisturization: Keeping the area well-moisturized can help alleviate dryness and reduce itching. Use a gentle, fragrance-free moisturizer or a silicone-based gel that can help hydrate the skin and improve its elasticity.
2. Topical Treatments: Over-the-counter hydrocortisone cream can help reduce inflammation and itching. Additionally, anti-itch creams containing ingredients like menthol or pramoxine can provide temporary relief.
3. Cold Compresses: Applying a cold compress to the area can help numb the sensation and reduce itching. This can be particularly effective if the itching is severe.
4. Avoid Irritants: Be cautious about using harsh soaps, lotions, or other products on the area. Opt for gentle, hypoallergenic products to minimize irritation.
5. Antihistamines: Oral antihistamines can help reduce itching caused by histamine release. However, consult with a healthcare provider before starting any new medication.
6. Physical Therapy: If the itching is related to nerve regeneration or scar tissue, physical therapy may help. A physical therapist can provide exercises and treatments to improve mobility and reduce discomfort in the area.
7. Consult a Dermatologist: If the itching persists or worsens, it may be beneficial to consult a dermatologist. They can assess the area for any underlying issues, such as infection or allergic reactions, and provide targeted treatments.
8. Mindfulness and Distraction Techniques: Sometimes, focusing on the sensation can exacerbate the feeling of itching. Techniques such as mindfulness, meditation, or engaging in distracting activities can help manage the perception of itching.
Conclusion
Deep itching after surgery, particularly in the area of a previous lipoma removal, can be attributed to nerve regeneration, scar tissue formation, dry skin, and histamine release. While it can be uncomfortable, there are several strategies to alleviate the itching, including moisturization, topical treatments, and possibly antihistamines. If the problem persists, seeking professional advice from a dermatologist or healthcare provider is advisable to rule out any complications and receive tailored treatment.
