Chest tightness, numbness in hands and feet, mouth sores, palpitations, insomnia?

Hello, doctor.
I have the following questions regarding possible causes and which specialty I should consult.
Thank you.
1.
I experience numbness, tingling, and a burning sensation in the joints of both hands, the back of the hands, palms up to the elbows, around the knees and the tops of the feet, ankles, and toes.
The symptoms are more severe at night, but I also experience them during the day when I am still.
(I have already seen a rehabilitation specialist who took X-rays and a rheumatologist who conducted blood tests, both of which showed no abnormalities.
There is a suspicion of carpal tunnel syndrome, and I have seen a neurologist who did not perform relevant tests.) Medication treatment: Neurology prescribed Chuangli coated tablets / Rehabilitation prescribed Ibuprofen tablets (I am taking both together, which provides slight improvement, but the symptoms return upon discontinuation of the medication).
2.
I have unexplained chest tightness, palpitations, insomnia, muscle tension, and fatigue.
3.
Occasionally, I experience diarrhea and lack of appetite.
4.
I have mouth sores and experience sudden sharp pain in my eyes, which resolves quickly.
5.
My body temperature is slightly elevated, around 37-37.5°C, but my extremities are usually cold.
6.
Occasionally, I experience stiffness in my neck on both sides.

Dear Ms.
Vivian,
You mentioned experiencing symptoms such as numbness, tingling, and a burning sensation around the finger joints, the back and palm of the hands up to the elbows, as well as around the knees, the tops of the feet, the ankles, and the toes.
These symptoms do not seem to be solely explainable by carpal tunnel syndrome or ulnar nerve compression.
It may be necessary to consider rarer metabolic disorders.
Nerve conduction studies and electromyography may be needed to assist in the diagnosis.
I recommend consulting a neurologist specializing in peripheral nerve disorders at a medical center, as they have more experience with electromyography diagnostics.
Thank you for your inquiry.
Feel free to discuss further.
Wishing you good health and peace,
Dr.
Jiang Junyi, Neurologist, Taoyuan Hospital

More Info

Your symptoms suggest a complex interplay of neurological, psychological, and possibly physiological factors. The combination of numbness, tingling, and sensations of heat in your extremities, particularly worsening at night, could indicate a peripheral neuropathy or a condition such as carpal tunnel syndrome, especially since you mentioned that you have seen a neurologist without conclusive results.
1. Neurological Symptoms: The numbness and tingling in your fingers, hands, and feet can be attributed to several potential causes. Peripheral neuropathy, which can arise from diabetes, vitamin deficiencies (especially B12), or autoimmune diseases, is a common culprit. Carpal tunnel syndrome, which you mentioned, occurs when the median nerve is compressed at the wrist, leading to symptoms in the hand and fingers. Given that you have already consulted with a neurologist and undergone imaging and blood tests, it may be beneficial to pursue nerve conduction studies or electromyography (EMG) to assess the function of your nerves and muscles more thoroughly.

2. Chest Tightness and Anxiety: The feelings of chest tightness, palpitations, and muscle tension can often be linked to anxiety disorders. Anxiety can manifest physically, leading to symptoms such as shortness of breath, heart palpitations, and muscle tightness. The fact that you experience these symptoms alongside sleep disturbances and fatigue suggests that anxiety may be a significant factor in your overall condition. Cognitive-behavioral therapy (CBT) and other forms of psychotherapy can be effective in managing anxiety symptoms.

3. Gastrointestinal Symptoms: Occasional diarrhea and loss of appetite can also be linked to anxiety or stress. The gut-brain connection is well-documented, and psychological stress can lead to gastrointestinal disturbances. Keeping a food diary to track what you eat and any subsequent symptoms may help identify any food intolerances or triggers.

4. Mouth Ulcers and Eye Pain: The occurrence of mouth sores and sudden eye pain that resolves quickly could indicate an underlying inflammatory condition or stress-related issues. Conditions like aphthous ulcers can be triggered by stress, while eye pain could be related to tension or strain, especially if you are experiencing anxiety.

5. Body Temperature and Cold Extremities: A slightly elevated body temperature combined with cold extremities can be perplexing. This could be a sign of an underlying infection or inflammatory process, but it could also be related to anxiety, which can cause peripheral vasoconstriction, leading to cold hands and feet.

6. Muscle Tension and Sleep Issues: The muscle tightness and sleep disturbances you describe are often seen in individuals with anxiety or stress-related disorders. It may be beneficial to explore relaxation techniques, such as mindfulness meditation, yoga, or progressive muscle relaxation, to help alleviate these symptoms.

Given the complexity of your symptoms, it would be advisable to consult a multidisciplinary team, including a neurologist, psychiatrist, and possibly a rheumatologist, to explore all potential underlying causes. A thorough evaluation can help rule out any serious conditions and guide you toward appropriate treatment options.
In summary, your symptoms could be indicative of a combination of neurological issues, anxiety, and possibly gastrointestinal disturbances. A comprehensive approach involving both medical and psychological evaluations may provide the best pathway to understanding and managing your condition effectively.
