Sharp pain in the chest, sore throat, and phlegm?

Hello Doctor: I would like to ask about a recent sudden increase in phlegm in my throat, along with a tight sensation.
When I press near my lower jaw while swallowing saliva, it hurts, and I also experience sharp pain when pressing on my chest.
I initially thought it was a cold and have taken medication for several days without improvement.
I'm unsure what the issue might be.
Should I see a pulmonologist, an otolaryngologist, or a cardiologist? Thank you.

Dr. Wang Lanfeng reply Pulmonology

Hello: Based on your description, your symptoms are more indicative of an upper respiratory infection.
I recommend that you visit a pulmonology clinic for further evaluation.
Thank you.

More Info

Chest pain and throat discomfort can be concerning symptoms that may arise from various underlying conditions. Given your description of symptoms—such as increased phlegm in the throat, tightness, pain when swallowing, and sharp pain in the chest—it's essential to approach this situation with caution and seek appropriate medical evaluation.

When to See a Specialist
1. Immediate Evaluation: If you experience severe chest pain, especially if it radiates to your arm, jaw, or back, or if you have associated symptoms like shortness of breath, sweating, nausea, or dizziness, you should seek emergency medical attention immediately. These could be signs of a heart attack or other serious conditions.

2. Persistent Symptoms: Since you mentioned that your symptoms have not improved despite taking medication for several days, it is advisable to see a healthcare professional. Persistent symptoms warrant further investigation to determine the underlying cause.

3. Choosing the Right Specialist:
- Ear, Nose, and Throat (ENT) Specialist: Given your throat symptoms, including the feeling of tightness and pain when swallowing, an ENT specialist would be a good starting point. They can evaluate for conditions such as pharyngitis, laryngitis, or other throat-related issues that could be causing your discomfort.

- Pulmonologist (Chest Specialist): If your symptoms include significant chest pain or if there is a concern about lung-related issues (like pneumonia or pleuritis), a pulmonologist may be appropriate. They can conduct tests such as chest X-rays or CT scans to assess lung health.

- Cardiologist (Heart Specialist): If there is any suspicion that your chest pain could be cardiac in nature, a cardiologist should be consulted. They can perform an electrocardiogram (ECG) and other tests to evaluate heart function.

Possible Causes of Your Symptoms
- Musculoskeletal Issues: Sometimes, chest pain can arise from muscle strain or inflammation in the chest wall, which can also cause pain when pressing on the chest.

- Gastroesophageal Reflux Disease (GERD): Acid reflux can cause a sensation of tightness in the throat and chest pain, particularly after eating or when lying down.

- Infections: Viral or bacterial infections can lead to throat discomfort and may also cause referred pain in the chest area.

- Anxiety or Stress: Psychological factors can manifest as physical symptoms, including chest pain and throat tightness.

Recommendations
1. Document Symptoms: Keep a record of your symptoms, including when they occur, their intensity, and any associated factors (like eating, stress, or physical activity). This information can be helpful for your healthcare provider.

2. Seek Medical Attention: Given the persistence of your symptoms and their impact on your daily life, it is advisable to schedule an appointment with an ENT specialist first. They can assess your throat and determine if further evaluation by a pulmonologist or cardiologist is necessary.

3. Avoid Self-Diagnosis: While it can be tempting to self-diagnose based on symptoms, it is crucial to have a professional evaluation to rule out serious conditions.

In conclusion, your symptoms warrant a thorough evaluation by a healthcare professional. Starting with an ENT specialist seems appropriate given your throat discomfort, but be prepared to follow up with other specialists if necessary. Early intervention can lead to better outcomes, so don’t hesitate to seek help.
