Swelling and redness below the left zygomatic bone?

Hello Doctor: Thank you for taking the time to help me with my concerns.
I have swelling and redness in the lower left zygomatic area, presenting a rounded appearance (approximately 2 cm in diameter), and it appears uniformly red rather than spotted.
It feels slightly firm to the touch.
Over the past three to four months, it has occasionally turned red for 1 to 2 days before returning to normal.
I am reaching out specifically because this time the symptoms are particularly red and swollen, and on the day of the recurrence, I also experienced itching on my forehead, along with itching in the lower left zygomatic area.
I would like to ask the doctor about the possible causes.
Should I seek medical attention?

Dr. Wu Yuxin reply Dermatology

Is it possible that it is angioedema? Without seeing the lesions, it is difficult to determine the issue.
If it is very concerning, please seek medical attention promptly.

The symptoms you are describing regarding the red swelling below your left cheekbone could be attributed to several potential causes, and it is important to consider the characteristics of the swelling, its duration, and any associated symptoms. Based on your description, the swelling appears to be a round, uniformly red area that feels slightly hard to the touch. Additionally, you mentioned that it has been recurring over the past three to four months, with episodes lasting 1-2 days before resolving.

One possible cause of this swelling could be a dermatological condition such as an allergic reaction or dermatitis. The itching you experience on your forehead and the area around your cheekbone may suggest an allergic response, possibly to a topical product, food, or environmental allergen. Allergic reactions can manifest as localized swelling, redness, and itching, and they can vary in intensity and duration.

Another possibility is that the swelling could be related to a benign cyst, such as an epidermoid cyst or a sebaceous cyst. These cysts can become inflamed or infected, leading to redness and swelling. They are typically firm to the touch and can fluctuate in size. If the cyst becomes infected, it may also present with additional symptoms such as pain, warmth, and possibly drainage of pus.

In some cases, the swelling could be indicative of a more serious condition, such as an infection (like cellulitis) or an inflammatory process (such as an autoimmune condition). If the swelling is accompanied by systemic symptoms such as fever, increased pain, or spreading redness, it would be essential to seek medical attention promptly.

Given that your symptoms have persisted and are particularly pronounced this time, it is advisable to consult a healthcare professional for a thorough evaluation. A dermatologist or an ear, nose, and throat (ENT) specialist would be well-suited to assess your condition. They may perform a physical examination and, if necessary, order imaging studies or a biopsy to determine the exact nature of the swelling.

In summary, while there are several potential causes for the red swelling below your left cheekbone, including allergic reactions, cysts, or infections, it is crucial to seek medical advice for an accurate diagnosis and appropriate treatment. Early intervention can help prevent complications and provide relief from your symptoms.
