Linear Shockwave Therapy

Hello, doctor.
This is my report after undergoing shockwave therapy for the cavernous body.
Doppler blood flow report:
Right cavernosal artery diameter: 0.8 mm.
Velocity: 0.58 m/sec; RI: 1.0
Left cavernosal artery diameter: 0.9 mm.
Velocity: 0.63 m/sec; RI: 1.0
I sought medical help due to sudden erectile dysfunction and visited several hospitals.
Eventually, at Linkou Chang Gung Memorial Hospital, I was diagnosed with low cavernous blood flow, which was only 11.
The doctor recommended extracorporeal shockwave therapy.
After the treatment, I had another Doppler assessment, and the results are as above.
The doctor said the blood flow is now normal, but I still experience difficulties during masturbation and sexual intercourse.
Why is this the case? The report is normal, and I do not have any metabolic syndrome, yet I still rely on Cialis to complete sexual activity.
I am still young and hope to find the real cause.
I also asked the doctor who treated me, and I have undergone shockwave therapy six more times after the initial 14 sessions.
The doctor mentioned it might be a psychological issue.
What should I do? Is this related to long-term masturbation? I masturbate approximately 1-3 times a day.

Dr. Xu Wencang reply Urology

At 26 years old, there should be no significant organ issues.
It may be beneficial to focus on psychological adjustment and maintain a normal lifestyle.
Occasionally using medications like Cialis for assistance is also an option.

Understanding Linear Shockwave Therapy for Erectile Dysfunction (ED) involves recognizing both the physiological aspects of erectile function and the psychological factors that can contribute to sexual performance issues.
Erectile dysfunction is often linked to inadequate blood flow to the penis, which can be caused by various factors, including vascular health, hormonal levels, and psychological well-being. In your case, the report indicates that after undergoing Linear Shockwave Therapy, the blood flow parameters improved significantly, suggesting that the treatment was effective in enhancing blood circulation in the cavernosal arteries. The measurements you provided show that both the right and left cavernosal arteries have normal diameters and velocities, which is a positive sign.

However, despite these improvements in blood flow, you are still experiencing difficulties with sexual performance, which can be frustrating. It’s important to understand that erectile function is not solely dependent on physical factors; psychological components play a significant role as well. Anxiety, stress, and performance pressure can all contribute to ongoing erectile dysfunction, even when physiological parameters appear normal. This phenomenon is often referred to as "psychogenic ED," where the mind influences the body's ability to perform sexually.

Regarding your concern about the frequency of masturbation, while masturbation itself is a normal sexual activity, excessive or compulsive masturbation can sometimes lead to psychological issues or performance anxiety. If you find that your sexual experiences are becoming less satisfying or that you are relying on specific conditions (like medication) to achieve an erection, it may be beneficial to explore these feelings further.
Here are some suggestions to consider:
1. Psychological Counseling: Engaging with a therapist who specializes in sexual health can help address any underlying anxiety or psychological barriers that may be affecting your sexual performance. Cognitive-behavioral therapy (CBT) is particularly effective for performance anxiety.

2. Open Communication: If you have a partner, discussing your feelings and experiences openly can alleviate some of the pressure you may be feeling. A supportive partner can help create a more relaxed environment for intimacy.

3. Gradual Exposure: Instead of focusing solely on achieving an erection, try to shift your focus to the overall experience of intimacy. This can help reduce performance pressure and allow you to enjoy the moment without the stress of expectations.

4. Lifestyle Modifications: While you mentioned that you do not have high blood pressure or other metabolic issues, maintaining a healthy lifestyle is crucial. Regular exercise, a balanced diet, and adequate sleep can all contribute to improved sexual health.

5. Follow-Up with Your Doctor: Since you have undergone multiple sessions of shockwave therapy, it’s essential to maintain communication with your healthcare provider. They can help monitor your progress and make any necessary adjustments to your treatment plan.

6. Limit Stimulants: If you are using medications like Viagra (sildenafil), ensure that you are using them as prescribed. Over-reliance on these medications can sometimes lead to psychological dependence, where you feel you cannot perform without them.

In conclusion, while the physiological aspects of erectile dysfunction are crucial, it’s equally important to address the psychological components. By combining medical treatment with psychological support and lifestyle changes, you can work towards achieving a more satisfying sexual experience. Remember, it’s a journey, and seeking help is a positive step toward understanding and overcoming these challenges.
