The issue of premature ejaculation?
Doctor, I have recently noticed issues with premature ejaculation over the past six months, with the duration being reduced by about half.
Additionally, I experience a slight burning sensation in the urethra before and after urination, but this pain only occurs during urination.
Could this be related to the finasteride 5mg that I have been taking for over a year, at a dosage of 0.25 daily? Thank you.

Hello: The package insert does not mention this side effect, but if there are abnormalities in urination, a medical examination should be sought, and at the very least, a urinalysis should be performed.
Wishing you good health.

Premature ejaculation (PE) and urinary pain can be distressing symptoms that significantly impact a person's quality of life. In your case, you have noted a recent onset of premature ejaculation and mild urethral pain during urination, and you are concerned about the potential link to finasteride, which you have been taking for over a year.
Finasteride is a medication primarily used to treat benign prostatic hyperplasia (BPH) and male pattern baldness. It works by inhibiting the enzyme 5-alpha-reductase, which converts testosterone to dihydrotestosterone (DHT). While finasteride is generally well-tolerated, it can have side effects, including sexual dysfunction. Some studies have reported that finasteride may contribute to issues such as decreased libido, erectile dysfunction, and, in some cases, premature ejaculation. However, the relationship between finasteride and PE is not fully understood and may vary from person to person.
Regarding your symptoms, the premature ejaculation you are experiencing could be multifactorial. Psychological factors such as anxiety, stress, or relationship issues can contribute to PE. Additionally, physical factors, including hormonal changes or nerve sensitivity, may also play a role. It is essential to consider that the onset of PE may not solely be attributed to finasteride, especially if you have been taking it for an extended period without previous issues.
The mild urethral pain you described during urination could be related to several conditions, including urinary tract infections (UTIs), urethritis, or prostatitis. Finasteride is not typically associated with causing urinary pain directly; however, it can affect prostate health, which may indirectly influence urinary symptoms. If you are experiencing persistent pain, it is crucial to consult a healthcare professional for a thorough evaluation. They may recommend a urinalysis or other diagnostic tests to rule out infections or other underlying conditions.
If you suspect that finasteride may be contributing to your symptoms, it is advisable to discuss this with your prescribing physician. They can assess your situation and determine whether a dosage adjustment or a change in medication is appropriate. It is important not to discontinue any medication without professional guidance, as this could lead to unintended consequences.
In summary, while finasteride may have some association with sexual dysfunction, including premature ejaculation, it is essential to consider other potential factors contributing to your symptoms. A comprehensive evaluation by a healthcare provider is necessary to identify the underlying causes of your premature ejaculation and urinary pain. They can provide tailored advice and treatment options to help manage your symptoms effectively.
