Could COVID Vaccine Trigger Arthritis Flare-Ups Months Later? - Internal Medicine

Will the recurrence of the condition after receiving the COVID-19 vaccine occur only three months later?

Hello Doctor, I am a patient with ankylosing spondylitis.
I suspect that after receiving the first dose of the BNT vaccine, my previously well-controlled ankylosing spondylitis has relapsed (which is why I am hesitant to get the second dose).
However, the relapse began three months after the vaccination, so I would like to ask the doctor, is there a high "possibility" that this relapse is caused by the vaccine, given that it started three months after receiving the COVID-19 vaccine? Thank you, Doctor.

Jie, 10~19 year old female. Ask Date: 2022/05/20

Dr. Liao Zhongxin reply Internal Medicine

The concern regarding whether a COVID-19 vaccine could trigger arthritis flare-ups, particularly in patients with pre-existing conditions like ankylosing spondylitis, is a valid one. While there is no definitive answer, it is important to consider several factors based on current medical understanding.

Firstly, vaccines, including the COVID-19 vaccines, are designed to stimulate the immune system to recognize and fight off infections. In some individuals, especially those with autoimmune conditions, this immune response can potentially lead to a flare-up of their underlying condition. However, it is essential to note that this is not a common occurrence and varies significantly among individuals.

In your case, experiencing a flare-up of ankylosing spondylitis three months after receiving the first dose of the BNT vaccine raises questions about the relationship between the vaccine and your symptoms. While it is theoretically possible that the vaccine could have contributed to the flare-up, it is also important to consider other factors that might have influenced your condition during that time. For instance, stress, changes in physical activity, or even other infections could also trigger a flare-up of ankylosing spondylitis.

Research has shown that some patients with autoimmune diseases may experience exacerbations after vaccination, but these instances are generally rare. A study published in the journal "Rheumatology" indicated that while some patients with autoimmune diseases reported flare-ups post-vaccination, the benefits of vaccination in preventing severe COVID-19 illness typically outweigh the risks of potential flare-ups.

Moreover, the timing of your flare-up is also significant. The immune response to a vaccine can take time to manifest, and it is not uncommon for individuals with autoimmune conditions to experience fluctuations in their symptoms due to various triggers unrelated to vaccination. Therefore, attributing your flare-up solely to the vaccine may not be entirely accurate without further investigation.

It is crucial to discuss your concerns with your healthcare provider, who can assess your specific situation, review your medical history, and possibly recommend further evaluation or adjustments to your treatment plan. They may also suggest monitoring your symptoms closely after the second dose of the vaccine, should you choose to proceed with it.

In conclusion, while there is a possibility that the COVID-19 vaccine could trigger a flare-up of ankylosing spondylitis, the likelihood of this occurring varies among individuals. It is essential to weigh the potential risks against the benefits of vaccination, especially given the ongoing threat of COVID-19. Engaging in an open dialogue with your healthcare provider will help you make an informed decision about your vaccination and management of your condition.
