Upon waking up, you may see many black lines?
Hello, recently I have been experiencing black lines filling my vision every morning as soon as I wake up, which quickly disappear.
Sometimes, I also notice them when my eyes are half-closed, but they don't resemble the shapes associated with floaters; instead, they are semi-transparent.
I have high myopia of -7.25 diopters and dry eye syndrome.
I am currently taking iron supplements prescribed by my doctor and have no other chronic illnesses.
I have undergone a fundus examination, and the doctor said everything is fine, but the symptoms persist.
Can you please explain what might be causing this? Should I undergo any further examinations?

Dr. Gao Pilin reply Ophthalmology
Hello: Floaters are generally not something that will disappear.
If the doctor has examined you and found no issues, you can feel reassured to coexist with them.
They may persist for a long time, but you will get used to them, so please don't worry.

More Info
Seeing black lines upon waking can be a concerning symptom, especially for someone with a history of high myopia and dry eye syndrome. While you have already undergone an eye examination that showed no significant issues, it’s important to understand the potential causes of these visual disturbances and what steps you can take moving forward.
Possible Causes
1. Visual Phenomena: The black lines you are seeing could be a form of visual phenomenon known as "visual snow" or "entoptic phenomena." These are often harmless and can occur due to the way light interacts with the structures in your eye, particularly when transitioning from darkness (sleep) to light (waking).
2. Vitreous Changes: Given your high myopia, you may be more susceptible to changes in the vitreous gel that fills the eye. As we age or due to certain conditions, the vitreous can become more liquid and pull away from the retina, leading to symptoms like floaters or lines. This is often referred to as vitreous detachment, which can be more pronounced in individuals with high myopia.
3. Dry Eyes: Since you have dry eye syndrome, this could also contribute to visual disturbances. When the eyes are dry, the quality of the tear film is compromised, which can lead to blurred or distorted vision, especially upon waking when tear production may be lower.
4. Retinal Issues: Although your previous eye exam indicated no problems, it’s crucial to remain vigilant. Symptoms like black lines can sometimes indicate retinal issues, such as tears or detachments, especially in high myopes. Regular monitoring is essential.
Recommendations
1. Follow-Up with an Eye Specialist: Since your symptoms persist, it would be wise to schedule a follow-up appointment with your eye doctor. Discuss your symptoms in detail, including their frequency, duration, and any other visual changes you may have noticed.
2. Comprehensive Eye Exam: Request a comprehensive eye exam that includes a dilated fundus examination. This allows the doctor to get a better view of the retina and vitreous to rule out any potential issues that may not have been visible during your last visit.
3. Dry Eye Management: Since you have dry eye syndrome, consider discussing treatment options with your doctor. This may include artificial tears, prescription medications, or lifestyle changes to improve tear production and eye comfort.
4. Monitor Symptoms: Keep a log of when these black lines appear, their duration, and any associated symptoms (like headaches or changes in vision). This information can be helpful for your eye care provider in diagnosing the issue.
5. Lifestyle Adjustments: Ensure you are taking regular breaks from screens, staying hydrated, and using a humidifier if you are in a dry environment. These can help alleviate dry eye symptoms.
Conclusion
While seeing black lines upon waking can be alarming, it is often related to benign causes, especially in the context of your existing eye conditions. However, persistent symptoms warrant further investigation to rule out any serious underlying issues. Regular eye exams and open communication with your eye care provider are key to maintaining your eye health and addressing any concerns you may have.
