Why Is Throat Pain from COVID-19 So Hard to Relieve? - Otolaryngology

Is the throat inflammation caused by COVID-19 unmanageable in terms of pain relief?

Hello, Doctor.
I am currently a confirmed case and am in isolation, so I am seeing a doctor via telemedicine.

On the first day of my diagnosis, I had a fever all day and a swollen throat, so I consulted a pediatrician and received medication.
The fever subsided the next day.

On the second day of my diagnosis, my throat started to hurt, and by evening, the pain intensified.
The medication prescribed by the pediatrician did not alleviate the pain.
That night, I noticed several raised bumps on the pharyngeal wall behind the uvula, leading me to suspect acute pharyngitis.

On the third day, I was referred to an otolaryngologist and reported the following symptoms: "Hiccups, pain when swallowing saliva and drinking water, a burning sensation when exhaling, pain so severe that I don't want to eat, and the pain is worst when I get up.
It feels like my ears are also slightly painful, and there are raised bumps on the pharyngeal wall." The doctor prescribed medication to relieve "moderate pain," but after taking it for a day (three doses), I still have not experienced any relief.
My questions are:
1.
Is it common for acute pharyngitis to be unresponsive to pain relief medications? I have taken medications from both the pediatrician and the otolaryngologist, and I have tried acetaminophen, but none have provided any relief.
It is extremely painful to eat, drink, or even swallow saliva.

2.
Would you recommend a follow-up appointment to change my medication? I am unsure if the pain medication is not strong enough or if acute pharyngitis typically causes prolonged pain before pain relief becomes effective.
Since I am in isolation, I can only seek your advice here.
Thank you, Doctor.

Hello, it is recommended to schedule a formal telemedicine appointment for an accurate diagnosis and medication.
Sometimes, it's not just about taking pain relievers; a proper diagnosis is needed to determine the appropriate treatment.
Wishing you a speedy recovery.

Throat pain associated with COVID-19 can be particularly challenging to manage for several reasons. As a confirmed COVID-19 patient experiencing severe throat pain, it's understandable to seek clarity on why standard pain relief methods may not be providing the expected relief.

1. Nature of COVID-19 Related Throat Pain: The throat pain you are experiencing is likely due to inflammation caused by the viral infection. COVID-19 can lead to acute pharyngitis or tonsillitis, where the tissues in the throat become inflamed and swollen. This inflammation can cause significant discomfort, making swallowing and even talking painful. The presence of bumps or lesions on the throat, as you described, may indicate a more severe inflammatory response, which can complicate pain management.

2. Pain Management Challenges: Acute pharyngitis can be resistant to standard pain relief medications, especially if the inflammation is severe. Over-the-counter pain relievers like acetaminophen (Tylenol) or ibuprofen (Advil) may not be sufficient to alleviate the pain if the underlying inflammation is significant. In some cases, stronger prescription medications, such as corticosteroids, may be necessary to reduce inflammation and provide relief.
3. Duration of Symptoms: It’s important to note that throat pain from viral infections, including COVID-19, can persist for several days to weeks. The body’s immune response to the virus can cause prolonged inflammation, which may not respond immediately to pain relief medications. This can lead to a frustrating experience where pain persists despite medication.

4. When to Seek Further Medical Attention: Given that you have already consulted with both pediatric and ENT specialists and are still experiencing significant pain, it may be advisable to seek further medical evaluation. If your current pain management regimen is not effective, a healthcare provider may consider adjusting your treatment plan. This could include prescribing stronger pain relief medications or anti-inflammatory drugs, or even considering other interventions like throat gargles with antiseptic solutions, which can provide symptomatic relief.

5. Hydration and Nutrition: While managing throat pain, it’s crucial to maintain hydration and nutrition. If swallowing is too painful, consider consuming softer foods or liquids that are easier to swallow, such as smoothies, soups, or broths. Staying hydrated is essential for recovery, and warm liquids can sometimes provide soothing relief to an inflamed throat.

6. Home Remedies: In addition to prescribed medications, some home remedies may help alleviate throat pain. Gargling with warm salt water, using throat lozenges, or consuming honey (if not contraindicated) can provide temporary relief. Humidifiers can also help keep the throat moist, reducing discomfort.

7. Monitoring Symptoms: Keep a close eye on your symptoms. If you notice any worsening of your condition, such as difficulty breathing, high fever, or the development of new symptoms, it is crucial to seek immediate medical attention.

In summary, throat pain from COVID-19 can be particularly difficult to manage due to the nature of the viral infection and the body's inflammatory response. If standard pain relief methods are ineffective, it is essential to consult with your healthcare provider for a reassessment of your treatment plan. Remember to prioritize hydration and nutrition, and consider supportive home remedies to help ease your discomfort.
