Suspecting that you may have obsessive-compulsive personality disorder (OCPD)?
Hello, Dr.
Lin.
Recently, I came across some videos about obsessive-compulsive disorder (OCD) online, and I feel that my behaviors are very similar to those of the individuals in the videos.
For example, I have developed an inexplicable fear of toilets over the past few years.
I find toilets extremely dirty, to the point where I am afraid to press the flush button with my hands.
I always close the toilet lid and use my foot to press the flush button.
When using public restrooms, I leave flushing as the last step; after pressing the flush button, I immediately rush out, fearing contamination from bacteria.
I also have similar concerns about the refrigerator and doorknobs, imagining that they harbor many germs, and I feel uncomfortable every time I touch them.
Additionally, I often feel uneasy when I see hair on the floor or grease on tables, prompting me to clean them immediately.
Regarding certain behaviors, I am very reluctant to delegate work or tasks to others because I always feel that I must do it myself to ensure it is done correctly.
If I cannot observe others doing it the same way I would, I prefer to handle it myself.
I also cannot use blankets or towels that others have used; I must wash them again before using them.
Even when showering, I continuously rinse myself, fearing that I might not have cleaned some areas properly, even though I know I am clean.
My workspace and room are very tidy, and colleagues and friends often ask me if I have OCD.
Initially, I dismissed their comments because I thought most people wouldn't intentionally seek to understand OCD; they just label someone as "particular" or "picky." However, after watching that video and reflecting on it, I realized that I might have these tendencies.
Everything I described has never troubled me, but I still feel compelled to perform these actions.
After completing these tasks, I do not feel that they are unnecessary or that I want to stop, nor do I experience feelings of guilt.
Could you please tell me if these symptoms indicate tendencies toward obsessive-compulsive personality disorder? Thank you, doctor.

Dr. Lin Mianzhang reply Psychiatry
Hello, Xiangxiang: The situations you described are mostly related to cleanliness standards.
The reluctance to delegate tasks to others is more indicative of obsessive personality traits, so whether it meets the criteria for Obsessive-Compulsive Personality Disorder (OCPD) is still debatable.
Regarding your cleanliness standards, if you do not feel distressed or emotionally affected by them, it likely does not indicate Obsessive-Compulsive Disorder (OCD).
However, if you do feel distressed, it is advisable to seek an evaluation from a psychiatrist.
Wishing you peace and health!

Based on your description, it seems you are experiencing behaviors that align with some characteristics of Obsessive-Compulsive Personality Disorder (OCPD), but it is essential to differentiate between OCPD and Obsessive-Compulsive Disorder (OCD), as they are distinct conditions.
OCPD is characterized by a chronic preoccupation with rules, orderliness, and control. Individuals with OCPD often have a strong need for perfectionism and may struggle to delegate tasks to others because they believe that their way of doing things is the only correct way. This can lead to rigidity and difficulty in interpersonal relationships. Your reluctance to use public restrooms, fear of germs, and insistence on cleanliness and order in your environment are indicative of this personality disorder.
On the other hand, OCD involves unwanted, intrusive thoughts (obsessions) that lead to repetitive behaviors (compulsions) performed to alleviate the anxiety caused by those thoughts. While you mentioned that your behaviors do not cause you distress or a desire to stop, which is more characteristic of OCPD, it is important to note that OCD can also manifest in similar cleanliness and orderliness behaviors, but typically with a significant level of anxiety and distress associated with them.
Your fear of germs and the need to wash or clean things repeatedly could be seen as obsessive-compulsive traits, but since you do not feel compelled to stop these behaviors or feel guilty about them, it leans more towards OCPD. The fact that you feel the need to perform these actions without distress suggests that they are more about your personality style rather than a disorder that causes significant impairment in your daily life.
It is also worth noting that many people exhibit traits of OCPD without meeting the full criteria for the disorder. If your behaviors are not interfering with your daily functioning or causing significant distress, you may not need treatment. However, if you find that these behaviors are starting to impact your relationships or work life, or if you feel a desire to change them, it might be beneficial to seek help from a mental health professional.
In terms of treatment, cognitive-behavioral therapy (CBT) is often effective for individuals with OCPD. This approach can help you understand and modify your thought patterns and behaviors, allowing for greater flexibility and less rigidity in your daily life. Additionally, discussing your concerns with a therapist can provide you with strategies to cope with any anxiety related to your fears of germs and cleanliness.
In conclusion, while you exhibit behaviors that may suggest tendencies toward OCPD, it is crucial to consult with a mental health professional for a comprehensive evaluation. They can help clarify your symptoms, provide a diagnosis if necessary, and discuss potential treatment options if you feel that your behaviors are becoming problematic. Remember, seeking help is a positive step towards understanding yourself better and improving your quality of life.
