Postoperative dizziness after cataract surgery?

Hello Doctor, my father underwent cataract surgery in early April.
Initially, he mentioned feeling slightly dizzy, possibly due to adjusting to the difference in vision between his two eyes.
However, over the past two weeks, his dizziness has worsened to the point where he is unable to walk and has been lying down almost all day.
He even nearly fell while trying to walk.
Is it possible to experience such symptoms? (My father did not have myopia prior to the surgery.) I would appreciate your clarification, as he has had follow-up appointments, but the doctor did not provide any explanations.
Thank you.

Dr. He Jianxian reply Ophthalmology

Theoretically, there should be no dizziness after cataract surgery.
It is recommended to consult an otolaryngologist to check if there is an issue with the vestibular system.

Post-cataract surgery dizziness can be a concerning issue, especially when it significantly affects a patient's ability to walk or perform daily activities. It is important to understand that while some degree of dizziness or visual disturbances can be expected after cataract surgery, persistent or severe symptoms warrant further investigation.

Causes of Dizziness After Cataract Surgery:
1. Visual Adjustment: After cataract surgery, the brain needs time to adjust to the new visual input. This adjustment period can lead to temporary dizziness or imbalance as the brain recalibrates its perception of depth and distance. However, this typically resolves within a few days to a week.

2. Changes in Vision: If the patient underwent surgery for cataracts, the clarity of vision can change dramatically. If the patient has not had significant vision issues before, the sudden change can cause disorientation and dizziness as they adapt to seeing clearly.

3. Vestibular Dysfunction: The inner ear plays a crucial role in balance. If there is any disruption in the vestibular system, which can sometimes occur due to positioning during surgery or post-operative inflammation, it may lead to dizziness.

4. Medication Side Effects: Post-operative medications, including pain relievers or anti-inflammatory drugs, can sometimes have side effects that include dizziness. It is essential to review any medications the patient is taking.

5. Underlying Health Issues: Conditions such as low blood pressure, dehydration, or other systemic issues can exacerbate dizziness. If the patient has any pre-existing conditions, they may need to be evaluated to rule out any complications.

6. Complications from Surgery: Although rare, complications such as infection, bleeding, or retinal detachment can occur after cataract surgery. These complications can lead to severe symptoms, including dizziness, and should be evaluated promptly.

Solutions and Recommendations:
1. Follow-Up Appointment: It is crucial for your father to have a follow-up appointment with his ophthalmologist. If he has already had one and the doctor did not provide satisfactory answers, consider seeking a second opinion. A thorough examination may be necessary to rule out complications.

2. Balance and Vestibular Therapy: If the dizziness persists, a referral to a physical therapist specializing in vestibular rehabilitation may be beneficial. These therapists can provide exercises to help improve balance and reduce dizziness.

3. Medication Review: Ensure that all medications are reviewed by a healthcare professional. If dizziness is a side effect of any medication, alternatives may be available.

4. Hydration and Nutrition: Encourage your father to stay well-hydrated and maintain a balanced diet. Dehydration can contribute to dizziness, especially in older adults.

5. Monitoring Symptoms: Keep a detailed log of your father's symptoms, including when they occur, their severity, and any associated factors (like changes in position or activity). This information can be invaluable for healthcare providers in diagnosing the issue.

6. Safety Precautions: Until the cause of the dizziness is determined, ensure that your father takes precautions to prevent falls. This may include using assistive devices, ensuring good lighting in his environment, and having someone nearby to assist him.

In conclusion, while some dizziness after cataract surgery can be normal, persistent or severe symptoms should not be overlooked. It is essential to communicate openly with healthcare providers and advocate for further evaluation if necessary. With appropriate assessment and intervention, most patients can find relief from their symptoms and regain their balance and confidence.
