Fungal infection
Hello Doctor, I often have issues with skin breakdown on my glans, and it sometimes bleeds.
I previously visited a urologist and used a medication called Meikang ointment, which improved my condition, but it hasn't completely resolved the issue.
I ensure proper hygiene by drying the area after using the restroom and using a hairdryer on a cool setting after showering, followed by applying the Meikang ointment.
While it helps a bit, it still hasn't fully healed.
Recently, the condition has worsened, with bleeding and pain when exposed to water.
This has been ongoing for quite some time, and I have also had blood tests at the hospital, which ruled out sexually transmitted infections.
What should I do?
Jason, 20~29 year old female. Ask Date: 2022/06/26
More Info
Persistent glans skin issues can be quite distressing and may stem from various underlying causes. Based on your description, it seems you are experiencing recurrent skin problems on the glans, including cracking, bleeding, and pain, especially when in contact with water. Here’s a comprehensive overview of potential causes, treatments, and solutions for your condition.
Possible Causes
1. Irritation or Allergic Reaction: Frequent exposure to soaps, detergents, or personal hygiene products can lead to irritation or allergic reactions. This is particularly relevant if you have recently changed any products.
2. Infections: While you mentioned that sexually transmitted infections (STIs) have been ruled out, other infections such as yeast infections (Candida) or bacterial infections can still occur. These can cause inflammation, redness, and discomfort.
3. Dermatitis: Conditions like contact dermatitis or eczema can affect the glans, leading to symptoms like dryness, cracking, and bleeding. These conditions can be exacerbated by environmental factors, such as cold and dry weather.
4. Lichen Sclerosus: This is a chronic skin condition that can cause white patches, thinning of the skin, and sometimes pain or bleeding. It is more common in men and can be mistaken for other conditions.
5. Phimosis or Balanitis: If the foreskin is too tight (phimosis), it can lead to inflammation of the glans (balanitis), resulting in pain and discomfort.
Treatment Options
1. Topical Steroids: Since you have previously used a topical steroid (Mei Kang), it may help reduce inflammation and promote healing. However, prolonged use of topical steroids can lead to skin thinning, so it’s essential to use them under medical supervision.
2. Moisturizers: Applying a gentle, fragrance-free moisturizer can help keep the skin hydrated and reduce irritation. Look for products specifically designed for sensitive skin.
3. Antifungal or Antibacterial Creams: If an infection is suspected, your doctor may prescribe antifungal or antibacterial creams to address the underlying issue.
4. Avoiding Irritants: Identify and avoid any potential irritants, including soaps, lotions, or laundry detergents that may be causing a reaction.
5. Proper Hygiene: Continue your practice of keeping the area clean and dry. However, be cautious with the use of blow dryers, as excessive heat can also irritate the skin.
6. Consult a Dermatologist: If the condition persists or worsens, it is crucial to see a dermatologist. They can provide a more accurate diagnosis and may perform a skin biopsy if necessary to rule out conditions like lichen sclerosus or other dermatological issues.
Additional Recommendations
- Avoid Self-Medication: While it may be tempting to try over-the-counter treatments, it’s best to consult a healthcare professional for a tailored approach.
- Monitor Symptoms: Keep a diary of your symptoms, noting any changes or triggers, which can be helpful for your doctor.
- Consider Lifestyle Factors: Stress, diet, and hydration can also impact skin health, so consider these factors in your overall management plan.
In conclusion, persistent glans skin issues can arise from various causes, and effective treatment often requires a multifaceted approach. It is essential to work closely with a healthcare provider to identify the underlying cause and develop an appropriate treatment plan tailored to your needs. If your symptoms continue to cause concern, do not hesitate to seek further medical advice.
