Left lower abdominal pain?
Hello Dr.
Chen: This morning around 5 AM, I suddenly experienced severe pain in my lower left abdomen near the groin.
Lying down worsens the pain, but walking provides some relief, although it still hurts.
This happened last month as well, but the pain was more tolerable then.
However, today the pain intensity has increased compared to before.
I have checked previous consultation cases and found none similar to mine.
Could you please advise on the possible causes? Thank you!
Fei Er, 50~59 year old female. Ask Date: 2022/06/28

Severe left lower abdominal pain can be alarming and may arise from various underlying conditions. Understanding the potential causes is crucial for determining the appropriate course of action. Given your symptoms, which include sudden and intense pain near the groin area, it is essential to consider several possibilities.
1. Gastrointestinal Issues: One of the most common causes of left lower abdominal pain is related to the gastrointestinal tract. Conditions such as diverticulitis, which is the inflammation of diverticula in the colon, can cause severe pain, especially in the lower left abdomen. Symptoms may include fever, changes in bowel habits, and tenderness in the affected area. Another possibility is irritable bowel syndrome (IBS), which can lead to cramping and discomfort, though it typically does not cause severe pain.
2. Urological Problems: The kidneys and urinary tract can also be sources of pain. A kidney stone, particularly if it is located in the left kidney, can cause severe, sharp pain that radiates to the lower abdomen and groin. This pain often comes in waves and may be accompanied by nausea or changes in urination. A urinary tract infection (UTI) can also cause discomfort in the lower abdomen, although it is usually accompanied by other symptoms such as burning during urination or frequent urges to urinate.
3. Gynecological Conditions: In females, left lower abdominal pain can be associated with gynecological issues such as ovarian cysts, ectopic pregnancy, or pelvic inflammatory disease (PID). Ovarian cysts can cause pain if they rupture or become large, while ectopic pregnancies can lead to severe pain and require immediate medical attention. PID, an infection of the reproductive organs, can also present with lower abdominal pain, often accompanied by fever and unusual discharge.
4. Musculoskeletal Pain: Sometimes, pain in the lower abdomen can be referred pain from the back or pelvis. Conditions such as muscle strains or ligament injuries can lead to discomfort that may be felt in the abdominal area. Given that you mentioned the pain worsens when lying down, it could be related to muscle tension or strain.
5. Other Considerations: Other potential causes include hernias, which can present as a bulge and pain in the groin area, and appendicitis, although this typically presents with pain in the right lower abdomen. In rare cases, conditions such as inflammatory bowel disease (IBD) or even tumors can cause similar symptoms.
Given that your pain has increased in intensity and has occurred previously, it is crucial to seek medical evaluation. While some causes of abdominal pain may resolve on their own, others may require urgent treatment. A healthcare provider may recommend imaging studies such as an ultrasound or CT scan to identify the underlying cause. Blood tests may also be performed to check for signs of infection or inflammation.
In summary, while there are numerous potential causes for severe left lower abdominal pain, the exact diagnosis can only be made through a thorough medical evaluation. It is advisable to seek medical attention promptly, especially since the pain has escalated and is affecting your daily activities. Early diagnosis and treatment can significantly improve outcomes and alleviate discomfort.
