Suddenly, it hurts a lot?

Hello doctor, this morning I suddenly experienced severe pain in my lower left abdomen, and it also felt crampy when I pressed on it.
It was really painful...
I was awakened by the pain, and since it was only a little after six, no stores were open.
I thought about taking some Tylenol because the pain was so intense that I couldn't sleep and I was sweating.
After taking it, I felt better.
Someone told me it might be appendicitis, but I have my doubts because appendicitis usually causes pain in the lower right abdomen.
What could I possibly be suffering from?

Xiao 7, 20~29 year old female. Ask Date: 2005/05/11

Dr. Yuan Hongdao reply Gastroenterology and Hepatology

Hello, common causes of left lower abdominal pain include colitis, irritable bowel syndrome, ureteral stones, and issues related to the uterus and ovaries (such as ectopic pregnancy, endometriosis, or ovarian cysts).
If the pain persists, it is advisable to consult a gastroenterologist and a gynecologist.
Appendicitis is unlikely, as the appendix cannot be located on the left side except in rare cases of congenital organ situs inversus.
Additionally, appendicitis typically presents with continuous pain, mild fever, and loss of appetite.
In summary, if abdominal pain continues, medical attention is necessary.
Sincerely, Dr.

More Info

Sudden severe abdominal pain can be alarming, and it’s essential to understand the potential causes and whether they might indicate a serious condition. Based on your description of sudden, intense pain in the left lower abdomen, there are several possibilities to consider.

1. Diverticulitis: This is one of the most common causes of left lower abdominal pain, especially in adults. Diverticulitis occurs when small pouches (diverticula) in the colon become inflamed or infected. Symptoms often include sudden pain, fever, and changes in bowel habits. The pain is typically localized to the left side of the abdomen.

2. Kidney Stones: Given your history of a suspected kidney stone, it’s possible that you could be experiencing renal colic, which is severe pain caused by a stone obstructing the urinary tract. This pain often radiates from the back to the lower abdomen and can be accompanied by nausea and sweating.

3. Gastrointestinal Issues: Conditions such as irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), or even constipation can lead to significant abdominal pain. If you have a history of gastrointestinal issues, this could be a contributing factor.

4. Musculoskeletal Pain: Sometimes, pain in the abdomen can be related to muscle strain or injury. If you have been physically active or have lifted something heavy recently, this could be a possibility.

5. Gynecological Issues: In females, conditions such as ovarian cysts, ectopic pregnancy, or pelvic inflammatory disease can cause severe abdominal pain. If you are of reproductive age, these should be considered.

6. Appendicitis: While appendicitis typically presents with pain in the right lower abdomen, in some cases, especially in individuals with anatomical variations, it can present with pain on the left side. However, this is less common.

7. Hernia: An inguinal or femoral hernia can cause localized pain and discomfort, especially if there is a strangulation of the tissue.

Given that your pain was severe enough to wake you from sleep and caused sweating, it is crucial to take this seriously. While the pain subsided after taking medication, this does not rule out the possibility of a significant underlying issue.

Recommendations:
- Seek Medical Attention: It is advisable to consult a healthcare professional as soon as possible, especially if the pain returns or worsens. A physical examination, along with imaging studies such as an ultrasound or CT scan, may be necessary to determine the cause of your pain.

- Monitor Symptoms: Keep track of any additional symptoms such as fever, changes in bowel habits, nausea, or vomiting, as these can provide important clues to your healthcare provider.

- Avoid Self-Diagnosis: While it’s helpful to gather information, self-diagnosing can lead to unnecessary anxiety or delay in treatment.
In summary, while there are several potential causes for your left lower abdominal pain, it is essential to have a thorough evaluation to rule out serious conditions. Early diagnosis and treatment can significantly improve outcomes and alleviate discomfort.
