Persistent bleeding after hemorrhoid ligation?
I have three internal hemorrhoids.
The first one bled only once after a ligation procedure, and it was a significant amount of blood in the toilet.
After that, there were no issues.
Two weeks later, I had the second one treated, and now, after a week, I have had three bowel movements, all of which involved bleeding, with a significant amount of blood in the toilet.
During these bowel movements, I also experienced a strong sensation of friction.
I was originally scheduled to return for a follow-up in a month, but I would like to know if I need to return for treatment immediately.
Generally, I do not feel any particular discomfort, but I do experience pain and anxiety during bowel movements.

Dr. Zhang Shuhao reply Surgery
Minor bleeding within one to two weeks is normal and is caused by the incomplete healing of the hemorrhoid's wound after it has fallen off.
It is recommended to continue using hemorrhoid suppositories and to take warm sitz baths.
If there is no significant bleeding, you can wait until the scheduled follow-up appointment to return for evaluation.

When it comes to persistent bleeding after hemorrhoid ligation, it is crucial to understand the potential implications and when to seek medical help. Hemorrhoid ligation is a common procedure used to treat hemorrhoids, particularly internal ones, by cutting off the blood supply to the hemorrhoid, causing it to shrink and eventually fall off. While some bleeding can be expected post-procedure, especially during bowel movements, persistent or significant bleeding warrants further evaluation.
In your case, experiencing bright red blood in the toilet after bowel movements, especially if it occurs multiple times and is accompanied by a strong sensation of friction or pain, is concerning. Bright red blood typically indicates that the source of the bleeding is from the lower gastrointestinal tract, which includes the rectum and anus. This could be due to several factors, including:
1. Incomplete Healing: After ligation, the tissue may still be healing, and any strain during bowel movements can lead to bleeding. If the ligation site is still inflamed or irritated, it may bleed more easily.
2. Anal Fissures: The sensation of friction and pain during bowel movements could indicate the presence of anal fissures, which are small tears in the lining of the anus. These can occur due to passing hard stools or straining, and they can cause significant pain and bleeding.
3. Recurrent Hemorrhoids: It is also possible that the ligation did not fully address the hemorrhoids, or new hemorrhoids could have developed, leading to ongoing symptoms.
4. Infection or Complications: Although less common, there is a possibility of infection or other complications following the procedure, which could lead to increased bleeding.
Given that you have experienced significant bleeding on three separate occasions since the second ligation, it is advisable to seek medical attention sooner rather than later. While it may have been initially planned for a follow-up appointment in a month, the presence of persistent bleeding and pain suggests that your condition should be evaluated by a healthcare professional.
When you visit your doctor, they will likely perform a physical examination and may recommend additional tests, such as a colonoscopy, to rule out other potential causes of bleeding. They may also assess the ligation sites to ensure there are no complications and provide treatment options to alleviate your symptoms.
In the meantime, here are some general recommendations to help manage your symptoms:
- Dietary Adjustments: Ensure you are consuming a high-fiber diet to soften your stools and reduce straining during bowel movements. This can include fruits, vegetables, whole grains, and plenty of fluids.
- Stool Softeners: Over-the-counter stool softeners may help ease the passage of stools and reduce discomfort.
- Warm Sitz Baths: Taking warm baths can help soothe the anal area and promote healing.
- Avoid Straining: Try to avoid straining during bowel movements, as this can exacerbate bleeding and pain.
In conclusion, while some bleeding can be expected after hemorrhoid ligation, persistent bleeding, especially if it is significant, should not be ignored. It is essential to communicate your symptoms to your healthcare provider as soon as possible to ensure appropriate management and to rule out any serious complications.
