Bleeding at the vaccination site?

Hello, a 7-year and 11-month-old child noticed a large bloodstain (about the size of a five-dollar coin) on the cotton ball at the injection site after receiving the second dose of the pediatric BNT vaccine.
This has never happened with previous vaccinations.
Could this indicate that the vaccine was injected into a blood vessel? Are there any potential side effects or increased risk of myocarditis?

Mr. Jiang, 40~49 year old female. Ask Date: 2022/07/02

Dr. Xiao Kaiwen reply Pediatrics

The current BNT COVID-19 vaccine for children aged 5-11 is administered via intramuscular injection into the deltoid muscle of the upper arm, rather than through intravenous injection.
The depth of this intramuscular injection does not pass through major blood vessels, so the theoretical risk of the vaccine being directly injected into a large blood vessel is very low.
I believe this phenomenon is likely due to the rupture of microcapillaries after the injection, combined with incomplete pressure from the cotton ball applied afterward.
This should not affect the efficacy of the vaccine or increase the risk of side effects, nor does it raise the risk of myocarditis as a side effect.

When a child experiences bleeding at the vaccine injection site, it can understandably cause concern for parents. In the case of your 7-year-old who had a significant amount of blood on the cotton ball after receiving the second dose of the BNT (Pfizer-BioNTech) vaccine, it is important to understand the context and implications of this occurrence.

First and foremost, the BNT vaccine is administered intramuscularly, specifically into the deltoid muscle of the upper arm. This method of injection is designed to ensure that the vaccine is delivered into the muscle tissue, where it can be effectively absorbed and elicit an immune response. The likelihood of accidentally injecting into a major blood vessel during this process is extremely low. The anatomy of the upper arm indicates that large blood vessels are not typically located in the area where the vaccine is injected. Therefore, it is more plausible that the bleeding you observed was due to minor trauma to small blood vessels (capillaries) in the skin or muscle rather than a direct injection into a larger blood vessel.

The bleeding itself, especially if it is localized and not accompanied by other symptoms such as swelling, severe pain, or systemic reactions, is generally not a cause for alarm. It is common for some children to experience minor bleeding or bruising at the injection site, particularly if the injection was administered quickly or if the child has sensitive skin. Applying gentle pressure with a clean cloth or cotton ball for a few minutes can help to stop the bleeding.
Regarding your concerns about potential side effects, including an increased risk of myocarditis (inflammation of the heart muscle), it is important to note that the occurrence of myocarditis following mRNA COVID-19 vaccinations, while documented, is primarily associated with specific demographics, particularly adolescent males, and typically occurs after the second dose. The bleeding at the injection site does not correlate with an increased risk of myocarditis. The benefits of vaccination in preventing COVID-19 and its complications far outweigh the risks of rare side effects.

If the bleeding persists, or if you notice any unusual symptoms such as excessive swelling, redness, or signs of infection (like fever or pus), it would be prudent to consult with your child's healthcare provider for further evaluation. They can provide reassurance and ensure that your child is recovering well from the vaccination.

In summary, while it is natural to worry about any unusual reactions following a vaccination, the bleeding you observed is likely a minor issue related to the injection technique rather than a serious complication. Vaccination remains a critical tool in protecting children from COVID-19, and the overall risk of serious side effects is very low. Always feel free to reach out to your healthcare provider with any concerns, as they can provide personalized guidance based on your child's health history and current condition.

Hello! Vaccines are administered via intramuscular injection, so in principle, the medication should not leak out. The bleeding is likely due to the rupture of microcapillaries in the skin. If the cotton pad was not pressed against the bleeding point when applied, it could result...
