Premature ejaculation?

I have phimosis, but I can manually retract it without any issues.
During an erection, it naturally retracts to the coronal ridge, but I can't fully tighten it without using my hands.
Because of this, my method of masturbation involves sliding the foreskin, which usually lasts about ten minutes.
However, when I use lubricant or condoms for masturbation, I pull the foreskin all the way back to allow direct friction between my hand and the coronal ridge, preventing the foreskin from moving back and forth.
This makes it very sensitive, unlike the sliding method, and I often feel the urge to ejaculate within ten to twenty seconds, and I ejaculate in less than a minute.
I would like to know if this indicates a congenital premature ejaculation issue.
😭😭 (I masturbate about once a day, sometimes twice, and I feel that my frequency is quite high, so why does this still happen?)

Hello, Diving Bell: You are not inherently experiencing premature ejaculation! Please do not be anxious.
The sensitivity you are experiencing is due to your immature glans not yet being accustomed to the external stimuli.
This can gradually improve with practice (similar to patients who experience heightened sensitivity after circumcision, but eventually adapt to the stimulation over time).
I hope this answers your question.

Premature ejaculation (PE) is a common condition that can affect men of all ages, and it can be influenced by a variety of factors, including psychological, biological, and anatomical aspects. Your question about whether your foreskin issues are related to premature ejaculation is quite relevant, as anatomical factors can indeed play a role in sexual function.

From your description, it seems that you have a condition known as phimosis, where the foreskin cannot be fully retracted over the glans (the head of the penis). While you mention that you can manually retract the foreskin, the inability to do so comfortably during an erection may contribute to heightened sensitivity in that area. This increased sensitivity can lead to quicker ejaculation during sexual activities, including masturbation.

The mechanism behind premature ejaculation often involves heightened sensitivity of the glans and the surrounding areas. When you use lubrication and a condom, the direct friction against the glans may be more intense than when you use the sliding motion of the foreskin, leading to a quicker sensation of ejaculation. This is not uncommon among men who have a tighter foreskin or those who experience increased sensitivity.

It's important to note that PE can also be influenced by psychological factors such as anxiety, stress, or performance pressure. If you feel anxious about your sexual performance or have concerns about your condition, this could exacerbate the issue. Additionally, frequent masturbation, while generally considered healthy, can sometimes lead to a conditioning effect where the body becomes accustomed to a certain type of stimulation, potentially affecting ejaculation timing.

Here are some considerations and potential approaches to address your concerns:
1. Consult a Urologist: Since you have concerns about your foreskin and its potential impact on your sexual function, it would be beneficial to consult a urologist. They can assess whether your foreskin condition is contributing to your sensitivity and ejaculation timing. In some cases, a minor surgical procedure, such as circumcision or a preputioplasty, may be recommended to alleviate phimosis and improve sexual function.

2. Desensitizing Techniques: There are various techniques and products designed to help manage sensitivity. These include desensitizing creams or sprays that can reduce sensation temporarily, allowing for longer-lasting sexual activity. However, it's essential to use these products under the guidance of a healthcare professional.

3. Behavioral Techniques: Techniques such as the "stop-start" method or the "squeeze" technique can help you gain better control over ejaculation. These methods involve practicing control during masturbation or sexual activity, allowing you to become more aware of your body's responses.

4. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through Kegel exercises can improve control over ejaculation. These exercises involve contracting and relaxing the muscles used to stop urination.

5. Psychological Support: If anxiety or stress is a contributing factor, speaking with a therapist or counselor who specializes in sexual health can be beneficial. They can help you address any underlying psychological issues that may be affecting your sexual performance.

6. Frequency of Masturbation: While masturbation is a normal and healthy activity, if you feel that your frequency is contributing to your concerns, consider moderating it. This may help reset your sensitivity levels and improve your control.

In conclusion, while your foreskin condition may play a role in your experience of premature ejaculation, it is likely not the sole factor. A comprehensive approach that includes medical evaluation, behavioral techniques, and possibly psychological support can help you manage and improve your situation. Remember, you are not alone in this, and many men experience similar challenges. Seeking professional guidance is a positive step toward finding a solution that works for you.
