Occasional Blood in Stool After Hemorrhoid Surgery - Surgery

Two months after hemorrhoid surgery, there are occasional instances of bloody stools?

Hello doctor, I underwent a hemorrhoidectomy for mixed hemorrhoids in early May.
During my follow-up appointment, the doctor mentioned that the wound has healed, but recently I have occasionally noticed blood in my stool (there is no blood in the toilet water).
My bowel movements are relatively soft, and it doesn't feel like an anal fissure.
I would like to know why there might still be blood in my stool occasionally after hemorrhoid surgery.
Is it possible that the hemorrhoids have recurred? However, is it really possible for them to recur so quickly, just two months after the surgery?

Dr. Zhang Shuhao reply Surgery

Two months after the surgery, the wound should be fully healed, and a recurrence within such a short time is unlikely.
At this point, bleeding may be due to residual hemorrhoidal tissue that was not completely excised during the procedure, or it could originate from the rectum or colon.
It is recommended to continue with warm sitz baths and the use of ointments.
If the bleeding does not improve, a follow-up appointment should be scheduled to assess whether further examination is necessary.

After undergoing hemorrhoid surgery, it is not uncommon for patients to experience occasional blood in their stool, even if the surgical site has healed. This phenomenon can be concerning, but understanding the underlying causes can help alleviate anxiety and guide appropriate management.

Firstly, it's important to recognize that the surgical procedure itself, while aimed at removing hemorrhoids, can lead to some residual irritation or inflammation in the anal area. This irritation may result in minor bleeding, especially if there is any straining during bowel movements, even if the stool is soft. The healing process can also involve the formation of scar tissue, which may be more sensitive and prone to minor bleeding during the initial recovery phase.

In your case, since you mentioned that the blood is only present on the stool and not in the toilet water, this suggests that the bleeding is likely originating from the rectal area rather than from higher up in the gastrointestinal tract. This localized bleeding could be due to several factors:
1. Residual Inflammation: After surgery, the tissues in the anal region can remain inflamed for some time. This inflammation can lead to minor bleeding, especially if there is any mechanical irritation during bowel movements.

2. Anal Fissures: Even if you feel that you do not have a fissure, the anal canal can still develop small tears or fissures due to the passage of stool. These can occur even with soft stools, particularly if there is any straining involved.

3. Recurrent Hemorrhoids: While it is less likely for hemorrhoids to recur so soon after surgery, it is not impossible. Factors such as diet, hydration, and bowel habits play a significant role in the development of hemorrhoids. If there is a tendency for constipation or straining, this could contribute to the recurrence of hemorrhoidal symptoms.

4. Other Conditions: Although less common, other conditions such as diverticular disease, inflammatory bowel disease, or even colorectal polyps can cause blood in the stool. However, these are less likely given your recent surgery and the nature of the bleeding.

Given these possibilities, it is advisable to monitor the situation closely. If the bleeding persists, increases in volume, or is accompanied by other symptoms such as pain, changes in bowel habits, or systemic symptoms like fever, it would be prudent to return to your healthcare provider for further evaluation. They may perform a physical examination and possibly recommend additional diagnostic tests, such as a colonoscopy, to rule out any other underlying issues.

In the meantime, to minimize the risk of further bleeding, consider the following recommendations:
- Maintain a High-Fiber Diet: Incorporate plenty of fruits, vegetables, and whole grains into your diet to ensure soft stools and reduce straining during bowel movements.

- Stay Hydrated: Drink plenty of fluids to help keep your stools soft.

- Avoid Straining: When you feel the urge to have a bowel movement, try to go as soon as possible to avoid straining.

- Use Stool Softeners: If necessary, over-the-counter stool softeners can help ease bowel movements.

In conclusion, while occasional blood in the stool after hemorrhoid surgery can be concerning, it is often due to benign causes related to the healing process or minor irritations. However, if you have any doubts or if the situation changes, do not hesitate to consult your healthcare provider for personalized advice and reassurance.
