Bleeding after the ligation of an internal hemorrhoid?

Hello Doctor: I previously underwent a hemorrhoid ligation surgery.
Within a week post-surgery, I experienced slight bleeding during bowel movements (there was blood on the toilet paper), but there has been no bleeding since then until today, which is almost 20 days later.
This morning, after a bowel movement, the stool was yellow with no blood, but there were two drops of blood in the toilet, and there was a small amount of blood (pinkish) on the toilet paper after wiping.
Could this indicate a problem with my rectum (as some online articles suggest that rectal cancer can cause intermittent bleeding), or could it be a recurrence of my hemorrhoids? Thank you.

A Zhi, 40~49 year old female. Ask Date: 2016/04/23

Dr. Yang Zhichao reply Gastroenterology and Hepatology

Dear Mr.
A-Zhi,
Based on your description, please return to the medical facility where your mole was previously ligated for a follow-up examination.
If there is no bleeding from the wound, please arrange for a colonoscopy.

Thank you for your inquiry.

Post-surgery bleeding after an internal hemorrhoid ligation can be concerning, especially when it involves blood in the stool or on toilet paper. It's important to understand the potential causes and when to seek further medical evaluation.

After undergoing a hemorrhoid ligation, it is not uncommon to experience some bleeding, especially within the first week post-operation. This bleeding is usually minor and can be attributed to the healing process of the ligated tissue. The presence of blood on toilet paper or in the toilet bowl, as you described, could be due to several factors:
1. Normal Healing Process: After a ligation procedure, the tissue around the hemorrhoids is healing. Some minor bleeding can occur as the ligated hemorrhoids shrink and the surrounding tissue adjusts. This is typically not a cause for alarm unless the bleeding is heavy or persistent.

2. Recurrent Hemorrhoids: It is possible for hemorrhoids to recur after treatment. If you are experiencing new symptoms or if the bleeding seems to be increasing, it may indicate that the hemorrhoids have returned or that new ones have developed.

3. Anal Fissures: Sometimes, the act of passing stool can cause small tears in the anal lining, known as anal fissures. These can lead to bleeding and are often accompanied by pain during bowel movements.

4. Other Rectal Issues: While less common, other conditions such as rectal polyps or, in rare cases, colorectal cancer can cause rectal bleeding. It is important to note that while the presence of blood can be alarming, it does not automatically indicate cancer. However, any new or unusual bleeding should be evaluated by a healthcare professional.

Given that you experienced some bleeding after a bowel movement but did not have blood in your stool for several days, it may not be an immediate cause for concern. However, the fact that you are still noticing blood, even if it is minimal, warrants further investigation. Here are some recommendations:
- Monitor Symptoms: Keep track of any additional symptoms, such as pain, changes in bowel habits, or the frequency and amount of bleeding. This information will be helpful for your healthcare provider.

- Consult Your Doctor: It is advisable to reach out to your healthcare provider to discuss your symptoms. They may recommend a physical examination or further diagnostic tests, such as a colonoscopy, to rule out any serious conditions.

- Diet and Hydration: Ensure you are consuming a high-fiber diet and staying well-hydrated to help soften stools and reduce straining during bowel movements, which can exacerbate bleeding.

- Follow-Up Care: If you have not had a follow-up appointment since your surgery, consider scheduling one. This will allow your doctor to assess your healing and address any concerns you may have.

In summary, while some bleeding after an internal hemorrhoid ligation can be normal, it is essential to remain vigilant and consult with your healthcare provider if you notice any changes or if the bleeding persists. Early intervention can help prevent complications and ensure proper healing.
