Pregnancy or menstruation?

Hello doctor, my menstrual cycle is about 28 days, lasting approximately five days from start to finish.
My last menstrual period began on June 15.
I had unprotected intercourse on June 16 and June 17.
Three days before the bleeding started, I experienced diarrhea and headaches, which continued until now (the third day, July 13).
I began bleeding on July 11, and the next day I had severe abdominal pain.
By today, the third day, the pain has diminished to a slight discomfort.
This bleeding is lighter than usual; the amount over the three days feels similar to what I typically experience on the third day of my period, and most of the time I need to exert pressure for the blood to flow.
On the first day, the bleeding started as pink mixed with brown, and on the second day, it was bright red with clots, but later it appeared brown on the sanitary pad.
This morning, on the third day, I used a pregnancy test, which showed one line, but I am still feeling uneasy.
I started my first job in mid-June, and I have been feeling quite anxious most of the time, often experiencing nausea and frequent insomnia.
I am generally a nervous person.
I wonder if the stress I have been under this month could be the reason for the unusual menstrual experience, but I am unsure if it could affect me so drastically.
Based on all of this, I would like to ask whether this bleeding is a menstrual period or a sign of pregnancy?

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the factors affecting the menstrual cycle or intermenstrual bleeding are numerous, including age, environment, lifestyle, stress, emotional instability, weight changes, medications, emergency contraceptive pills, diet, polycystic ovary syndrome, uterine or cervical lesions, infections, ovulation, pregnancy, and breastfeeding, among others.
In the early stages of pregnancy, there may not be specific symptoms, and subjective symptoms cannot be used for diagnosis.
It is recommended that if symptoms persist or worsen, you should consult a gynecologist to check for hormonal imbalances or other causes.
Thank you! Wishing you peace and health, Dr.
Zhang.

Based on your detailed description, it seems you are experiencing a range of symptoms that are causing you considerable concern regarding whether you are experiencing a normal menstrual cycle or the possibility of pregnancy. Let’s break down your situation and address your concerns.

Firstly, your menstrual cycle of approximately 28 days is considered regular, and the symptoms you are experiencing—such as diarrhea, headaches, and abdominal pain—can sometimes accompany the premenstrual phase or the onset of menstruation. The fact that you had unprotected intercourse on June 16 and 17 raises the possibility of pregnancy, especially since you mentioned that your last period started on June 15.

The bleeding you experienced on July 11, which was different from your usual menstrual flow, could be attributed to several factors. It is not uncommon for stress and anxiety, especially related to starting a new job, to affect your menstrual cycle. Stress can lead to hormonal fluctuations that may cause changes in the timing, flow, and characteristics of your period. The initial pinkish-brown discharge you described could indicate implantation bleeding if you were pregnant, but it could also simply be the beginning of your menstrual cycle, especially if it is lighter than usual.

The fact that you took a pregnancy test and received a negative result (one line) is a significant indicator. Home pregnancy tests are generally reliable when taken after a missed period, but they can sometimes yield false negatives if taken too early. If your period is indeed lighter and different than usual, it might be worth waiting a few days and taking another test, or consulting with a healthcare provider for a blood test, which can provide more definitive results.

Regarding your symptoms of anxiety, nausea, and insomnia, these can also be linked to stress and hormonal changes during your menstrual cycle. It’s essential to consider your mental and emotional well-being, as high levels of stress can exacerbate physical symptoms and lead to a cycle of discomfort. Engaging in relaxation techniques, such as deep breathing exercises, meditation, or gentle physical activity, can help alleviate some of the stress you are experiencing.

In summary, while the symptoms you are experiencing could be related to either your menstrual cycle or early pregnancy, the negative pregnancy test suggests that it is more likely to be related to your menstrual cycle, potentially influenced by stress. However, if your symptoms persist or worsen, or if you continue to have concerns about pregnancy, I strongly recommend consulting with a healthcare provider. They can perform a more thorough evaluation, including a physical examination and possibly a blood test, to provide you with clarity and peace of mind.

Lastly, it’s crucial to take care of your mental health during this time. If you find that anxiety is significantly impacting your daily life, consider reaching out to a mental health professional who can provide support and coping strategies. Remember, it’s okay to seek help when you need it.
