Finger swelling / rapid heartbeat

Hello Doctor, I have been experiencing some noticeable discomfort lately and I'm unsure where to seek medical attention.
I would appreciate some professional advice.
Firstly, for the past few days, I have been waking up with swelling and pain in the fingers of my right hand, particularly in the joints closest to the palm.
The symptoms are especially severe in the morning, but they tend to ease during the day with physical activity and work.
However, I still feel some discomfort when making a fist, and my right hand sometimes feels a bit numb.
Occasionally, the entire right side of my body feels uncomfortable.
I am concerned that this could be due to carpal tunnel syndrome, rheumatoid arthritis, or even a stroke causing unilateral numbness.

Additionally, I have had two recurrent infections of Group A Streptococcus in the past two months.
During my visits to the clinic, my blood pressure and heart rate have spiked above 100, with readings as high as 126 and 154.
I also find myself easily short of breath during physical activity, and I have had a history of elevated heart rates since childhood, often feeling unexplained palpitations or skipped beats.
The clinic doctor suggested that I get my thyroid function checked or see a cardiologist.

Could you please advise me on which specialty I should start with for these symptoms? Thank you!

More Info

Based on your symptoms and concerns, it is essential to approach your health issues systematically. You have described several symptoms, including swollen fingers, rapid heartbeat, and episodes of discomfort, which can be indicative of various underlying conditions. Here’s a breakdown of your symptoms and recommendations on how to proceed.

Symptoms Overview
1. Swollen and Painful Fingers: The swelling and pain in your fingers, particularly in the joints near the palm, could suggest several conditions. Common possibilities include:
- Carpal Tunnel Syndrome: This condition occurs when the median nerve is compressed at the wrist, leading to symptoms like pain, swelling, and numbness in the fingers.

- Rheumatoid Arthritis: An autoimmune disorder that can cause joint inflammation, pain, and swelling, particularly in the hands.

- Gout: A type of arthritis characterized by sudden, severe attacks of pain, swelling, and redness, often affecting the big toe but can also affect fingers.

- Peripheral Neuropathy: This could explain the numbness and discomfort in the right side of your body.

2. Rapid Heartbeat and Breathing Difficulties: The episodes of rapid heartbeat (tachycardia) and feelings of breathlessness can be concerning. These symptoms may arise from:
- Anxiety or Panic Attacks: Often manifesting as rapid heart rate and shortness of breath.

- Hyperthyroidism: An overactive thyroid can lead to increased heart rate and anxiety-like symptoms.

- Cardiac Issues: Conditions such as arrhythmias or other heart-related problems could also cause these symptoms.

3. Recent Throat Infections: The recurrent infections could contribute to systemic stress on your body, potentially exacerbating your heart rate and respiratory symptoms.

Recommended Steps
Given the complexity of your symptoms, it is advisable to start with a primary care physician (PCP) or a family doctor. They can conduct a thorough evaluation, including a physical examination and a review of your medical history. Here’s how you can proceed:
1. Initial Consultation: Schedule an appointment with your PCP. Discuss all your symptoms in detail, including the swelling in your fingers, rapid heartbeat, and any episodes of breathlessness.

2. Diagnostic Tests: Your doctor may recommend several tests, including:
- Blood Tests: To check thyroid function, inflammatory markers, and other relevant parameters.

- Electrocardiogram (ECG): To assess your heart's rhythm and detect any abnormalities.

- Imaging Studies: Such as X-rays or ultrasounds of your hands to evaluate joint conditions.

3. Referral to Specialists: Depending on the findings, your PCP may refer you to specialists:
- Rheumatologist: If arthritis or autoimmune conditions are suspected.

- Cardiologist: For further evaluation of your heart symptoms.

- Neurologist: If nerve-related issues are suspected.

4. Management of Symptoms: While waiting for your appointment, consider lifestyle modifications that may help alleviate some symptoms:
- Rest and Ice: For swollen fingers, resting the hand and applying ice can reduce swelling.

- Stress Management: Techniques such as deep breathing, meditation, or gentle exercise may help manage anxiety and improve overall well-being.

- Hydration and Diet: Ensure you are well-hydrated and consuming a balanced diet to support your immune system, especially after recent infections.

Conclusion
Your symptoms warrant a comprehensive evaluation to determine their cause and appropriate treatment. Starting with a primary care physician will help you navigate through the necessary tests and referrals to specialists. Remember, early intervention can lead to better management of your symptoms and overall health. Don’t hesitate to seek medical attention, as your well-being is paramount.
