Does taking Azithromycin and Quinine cause severe hair loss?

Hello doctor, I am a patient with systemic lupus erythematosus.
I am currently taking quinine (two tablets a day) and azathioprine (one tablet a day).
Recently, I have been experiencing increased hair loss.
I would like to know if hair loss is a side effect of quinine and azathioprine, or if it is due to the disease itself causing the severe hair loss?

Aya, 20~29 year old female. Ask Date: 2022/07/22

Dr. Gao Jiankai reply Internal Medicine

Hello, thank you for your question.
I believe that disease activity may potentially lead to hair loss; considering the side effects of medications, among the drugs you mentioned, quinine has a higher likelihood of causing hair loss.
To differentiate the cause, you can ask your physician to assess the current activity level of your disease.
If your disease is currently stable, then it would be reasonable to suspect the side effects of the medication, particularly quinine.
Of course, you can also directly ask your attending physician for their opinion on this matter.
Best regards from the Allergy, Immunology, and Rheumatology Department of Changhua Hospital, Ministry of Health and Welfare.

Hello! Thank you for your question regarding the potential side effects of Quinine and Hydroxychloroquine, especially in relation to hair loss. As a lupus patient, it’s understandable that you are concerned about any changes in your hair health, particularly when managing a chronic condition.

Quinine is primarily used to treat malaria and certain muscle cramps, while Hydroxychloroquine is commonly prescribed for autoimmune diseases such as lupus and rheumatoid arthritis. Both medications have distinct mechanisms of action and side effect profiles.

Regarding hair loss, Hydroxychloroquine is generally not known to cause significant hair loss. In fact, some studies suggest that it may have a protective effect on hair in patients with lupus. However, it’s important to note that every individual may react differently to medications, and some patients have reported hair thinning or loss while on Hydroxychloroquine. This could be due to a variety of factors, including the underlying disease itself, rather than the medication.

Quinine, on the other hand, has been associated with various side effects, but hair loss is not commonly reported. The more frequent side effects of Quinine include gastrointestinal disturbances, tinnitus, and visual disturbances. However, if you are experiencing increased hair loss, it’s essential to consider other factors that could be contributing to this issue.

In lupus patients, hair loss can often be attributed to the disease itself. Lupus can cause inflammation of the hair follicles, leading to a condition known as lupus erythematosus, which can result in hair thinning or loss. Additionally, stress, hormonal changes, and nutritional deficiencies can also play significant roles in hair health. It’s crucial to assess your overall health, including your diet, stress levels, and any other medications you may be taking, as these can all impact hair growth.

If you are experiencing significant hair loss, it is advisable to consult with your healthcare provider or a dermatologist. They can help determine whether your hair loss is related to your medications, your lupus condition, or other factors. They may also recommend treatments or lifestyle changes that could help improve your hair health.

In summary, while Hydroxychloroquine and Quinine are not typically associated with severe hair loss, individual reactions can vary. The underlying lupus condition itself may be a more significant factor in your hair loss. It’s essential to have an open discussion with your healthcare provider to explore all potential causes and find the best approach to manage your symptoms effectively. Thank you for reaching out, and I hope this information helps you in your journey toward better health!
